胆胃宁颗粒剂治疗胆胃不和型胆汁反流性胃炎临床疗效观察

Clinical effect of bile reflux gastritis with gall bladder and stomach deficiency type treated with Dan-Wei-Ning granules

目的观察胆胃宁颗粒剂治疗胆胃不和型胆汁反流性胃炎的临床疗效及对胃动素(MLT)的影响。方法采用随机按数字表法将60例患者分为对照组和观察组各30例,对照组采用多潘立酮片联合铝碳酸镁片治疗(多潘立酮片,3次/d,10mg/次;铝碳酸镁片,3次/d,1000mg/次);观察组采用江苏省南通市中医院自制颗粒剂——胆胃宁颗粒(批准文号:苏药制字Z04001991)泡服,3次/d,10g/次。两组疗程均为8周。对两组患者治疗前后主要临床症状进行评分;并比较治疗前后胃镜下胆汁反流改善情况;检测两组患者治疗前后血浆MLT的水平。结果观察组临床总有效率93.33%,对照组为66.67%,观察组优于对照组(P<0.01);观察组治疗后脘腹胀满、胃脘灼痛、嗳气、反酸、恶心、呕苦评分均明显低于对照组(P<0.05);治疗后胃镜下胆汁反流改善情况观察组明显高于对照组(P<0.05);治疗后观察组血浆MLT水平明显高于对照组,对比差异有统计学意义(P<0.01)。结论胆胃宁颗粒剂治疗胆胃不和型胆汁反流性胃炎能明显减轻患者的临床症状,减少胆汁反流,其作用机理可能与提高血浆MLT水平有关。

Objective To observe the efficacy of Dan - Wei - Ning granules on bile reflux gastritis of gall bladder and stomach deficiency type and investigate its effect on motilin（MLT）. Methods sixty patients with bile reflux gastritis were randomly divided into group and observation （30 patients for each group） by random number table. Patients in control orally received domperidone tablets （1 granule/time, tid） and hydrotalcite （2 granules/time, tid）. Based on the treatment of control group, the patients in ob- servation group were given with Dan- Wei -Ning granules（ approval number ：Su pharmacists word Z04001991 ）（ 1 pouch/time, tid）. The treatment course was 8 weeks for both groups. Main symptom scores were evaluated pre - post treatment. Two groups of patients after endoscopic bile reflux improvement compared; MLT plasma levels were detected before and after the patients. Re- sults The clinical total effective rate of 93.33 % ,66.67% in the control group, the observation group than the control group （P 〈 0.01 ） ; the observation group after treatment, abdominal distention, epigastric burning, heating, acid reflux, nausea, vomiting pain scores were significantly lower than the control group （ P 〈 0.05 ） ; after treatment efficacy endoscopic group was significantly high- er（P 〈 0.05 ） ; after treatment, the observation group was sighificantly higher plasma levels of MLT, compared to the difference was statistically significance（P 〈0.01 ）. Conclusion Dan -Wei -Ning granules and treatment of biliary and gastric bile reflux gastritis can reduce the clinical symptoms, the improvement of living patients, and significantly increased plasma levels in patients MLT.
