摘要
目的 探讨彩色多普勒超声(CDUS)与磁共振成像(MRI)在乳腺癌诊断中的应用价值。方法 收集2014年6月-2016年5月我院收治的130例乳腺病变患者的临床资料。对照病理检查结果,比较CDUS弹性成像与3.0 T MRI对乳腺癌的诊断符合率以及肿块、毛刺征、钙化检出情况。结果 MRI对乳腺良性、恶性病变的诊断准确率分别为73.3%和67.4%,CDUS弹性成像对乳腺良性、恶性病变的诊断准确率分别为74.6%和75.6%,均具有一定漏诊率。CDUS弹性成像对肿块的敏感性较高,但对钙化、毛刺征的敏感性不高;MRI对毛刺征和肿块的敏感性较高,但对钙化的敏感性低,两者单独应用各有优劣。CDUS弹性成像结合MRI能进一步提高乳腺癌患者的肿块、毛刺征以及钙化的检出率(χ^2=10.736、3.125、22.428,均P〈0.05)。结论 CDUS弹性成像和MRI单独用于诊断乳腺癌均具有一定的漏诊、误诊率,但将两者结合后,其对乳腺肿块、毛刺征、钙化的诊出率均明显提高,能为乳腺癌早期诊断提供参考。
Objective To explore the application value of color Doppler ultrasound (CDUS) and magnetic resonance imaging (MRI) in the diagnosis of breast cancer.Methods The clinical data of 130 patients with breast diseases admitted to our hospital from June 2014 to May 2016 were collected.Control pathological examination results,the diagnostic accuracy of breast cancer and mass,Mao Cizheng,calcification of CDUS elastography and 3.0 T MRI were compared. Results The diagnostic accuracy of MRI for benign and malignant breast lesions was 73.3% and 67.4%,respectively.The diagnostic accuracy of CDUS elastography in benign and malignant breast lesions was 74.6% and 75.6%,respectively, with a certain rate of missed diagnosis.CDUS elastic imaging was sensitive to the masses,but it was not sensitive to calcification and Mao Cizheng.MRI had high sensitivity to burr sign and mass,but had low sensitivity to calcification.There were own advantages and disadvantages in the two separate applications.CDUS elastography combined with MRI could further improve the detection rate of breast cancer patients,Mao Cizheng and calcification (χ^2=10.736,3.125,22.428,P〈 0.05).Conclusion CDUS elastography and MRI alone for the diagnosis of breast cancer have a certain rate of missed diagnosis and misdiagnosis,but after the combination of the two methods,the rate of diagnosis of breast mass,Mao Cizheng and calcification are significantly increased,it can provide reference for early diagnosis of breast cancer.
