核心稳定性训练结合Bobath疗法对脑卒中患者下肢功能和平衡能力的影响

EFFECTS OF CORE STABILITY TRAINING COMBINED WITH BOBATH THERAPY ON LOWER LIMB FUNCTION AND BALANCE ABILITY OF STROKE PATIENTS

目的:探讨核心稳定性训练结合Bobath疗法对脑卒中患者下肢功能和平衡能力的作用。方法:44例脑卒中患者随机分为对照组与观察组,每组22例,对照组给予常规康复训练,观察组在对照组的基础上使用核心稳定性训练和Bobath疗法,共进行8周。分别使用Fugl-Meyer运动功能量表(FMA)、躯干控制能力Sheikh评定量表、Berg平衡量表(BBS)和Holden步行功能分级评定患者治疗前后下肢运动功能、躯干控制能力、平衡能力和步行功能,使用卷尺测量简易腹内压。结果:两组患者治疗后的FMA评分、Sheikh评分、BBS评分、Holden评分均明显高于治疗前,腹内压明显低于治疗前(P<0.05);并且,观察组治疗后四种评分和腹内压的改善情况明显优于对照组(P<0.05)。结论:在常规康复训练的基础上联合应用核心稳定性训练和Bobath疗法可明显改善脑卒中患者的下肢功能和平衡能力。

Objective： To investigate the effects of core stability training combined with Bobath therapy on lower limb function and balance ability of stroke patients. Methods： 44 stroke patients were randomly divided into control group and observation group with 22 patients in each group. The patients in control group were given conventional rehabilitation training; While the patients in observation group were also given core stability training combined with Bobath therapy. FMA scale, Sheikh scale, BBS scale, Holden walking function grading were used to evaluate the lower limb motor function, trunk control ability, balance ability and walking function of patients before training and 8 weeks training; Tape to measure the simple intra-abdominal pressure. Results： The FMA score, Sheikh score, BBS score and Holden score of patients in 2 groups after training were obviously higher than that before training（P〈0. 05）; the intra-abdominal pressure was obviously lower（P〈0. 05）. Moreover, the four scores and intra-abdominal pressure of patients in observation group improved significantly better than control group（P〈0. 05）.Conclusions： Core stability training combined with Bobath therapy can obviously improve the lower limb function and balance ability of stroke patients on the basis of conventional rehabilitation training.
