CA19-9、CysC及Hcy联合检测对胰腺癌的诊断价值

The Diagnostic Value of Combined Detection of CA19-9,CysC and Hcy in the Diagnosis of Pancreatic Cancer

目的探讨CA19-9、CysC及Hcy联合检测对胰腺癌的诊断价值,为临床诊治提供参考依据。方法选取2014年2月至2016年1月我院收治的50例胰腺癌患者作为研究对象,选择同期我院收治的胰腺炎60例,及同期健康体检者60例作为对照组。检测三组患者的血清CA19-9、CysC及Hcy水平,比较各项指标单项及联合检测对胰腺癌的诊断效果。结果胰腺癌组患者的血清CA19-9、CysC及Hcy水平均显著高于胰腺炎组及对照组,差异有统计学意义(P<0.05)。对于胰腺癌的诊断,单项检测血清CA19-9的灵敏度和特异性(85.7%,77.3%)显著高于CysC(71.4%,63.6%)和Hcy(78.6%,68.2%),差异有统计学意义(P<0.05)。对于胰腺癌的诊断,血清CA19-9+CysC+Hcy三项联合检测的灵敏度和特异性(96.4%,95.5%)显著高于两项联合检测CA19-9+CysC(89.3%,86.4%)和CA19-9+Hcy(92.9%,91.0%),差异均有统计学意义(P<0.05)。结论血清CA19-9、CysC及Hcy联合检测对胰腺癌的诊断具有重要的临床意义,值得进一步深入研究。

Objective To explore the value of combined detection of CA19-9, CysC and Hcy in the diagnosis of pancreatic cancer, and to provide reference for clinical diagnosis and treatment. Methods From February 2014 to January 2016, 50 patients with pancreatic cancer were selected as the research objects. During the same period time 60 cases of pancreatitis and 60 cases of healthy subjects were selected as the control groups. The serum levels of CA19-9, CysC and Hcy were detected in all patients of three groups. The single and combined detection of various indexes were compared on the diagnosis of pancreatic cancer. Results The level of serum CA19-9, CysC and Hcy in pancreatic cancer group were significantly higher than those in pancreatitis group and normal group, the difference was statistically significant （ P 〈 0.05 ）. The sensitivity and specificity of CA19-9（85.7%, 77.3% ） was significantly higher than that of CysC（71.4%, 63.6% ） and Hcy （ 78.6%, 68.2% ）, the difference was statistically significant （ P 〈 0.05 ）. The sensitivity and specificity of three joint detection of serum CA19-9 ＋ CysC ＋ Hcy （96.4%, 95.5% ） were significantly higher than that of the two combined detection of CA19-9 ＋ CysC（89.3%, 86.4% ） and CA19-9 ＋ Hcy （92.9% ＋ 91% ）, the differences were statistically significant （ P 〈 0. 05 ）. Conclusion The combined detection of serum CysC, CA19-9 and Hcy might be more sensitive and specific in diagnosis of pancreatic cancer, and it is worthy of further study.
