摘要
目的探讨糖化血红蛋白(Hb Alc)、果糖胺(FMN)、1,5-脱水葡萄糖醇(1,5-AG)在糖尿病(DM)诊断中的应用价值,为糖尿病的临床诊断和治疗提供依据。方法选择2013年5月至2016年5月我院住院治疗的DM患者90例作为实验组,选取同期在我院体检的健康成年人90例作为正常对照组。收集两组受试者的体形资料,并检测其血清Hb Alc、FMN、1,5-AG水平,采用统计软件SPSS 19.0对所得数据进行分析。结果实验组患者的体重、BMI高于对照组,差异有统计学意义(P<0.05),而两组实验对象的身高、收缩压、舒张压之间比较,差异不具有统计学意义(P>0.05);实验组患者的血清Hb Alc、FMN明显高于对照组,而1,5-AG水平明显低于对照组,差异均具有统计学意义(P<0.05);Hb Alc、FMN、1,5-AG及三者联合诊断的灵敏度分别为67.78%、63.33%、83.33%、84.44%;而诊断特异性分别为94.44%、93.33%、90.00%、91.11%。结论 Hb Alc、FMN、1,5-AG可以有效的反映DM患者的高血糖状况,三者联合检测可以有效的提高DM的诊断率,为临床上及时诊断及治疗DM提供依据。
Objective To investigate the clinical value of glycosylated hemoglobin (HbAlc), fructosamine (FMN) and 1,5- dehydrated alcohol (1,5-AG) in the diagnosis of diabetes mellitus (DM), and to provide the basis for clinical diagnosis and treatment of DM. Methods We selected 90 cases of DM in our hospital as experimental group from May 2013 to May 2016, and 90 healthy adults were selected as normal control group. The body indexes of two groups of patients were collected, and the serum HbAlc, FMN and 1,5-AG level were detected, and the data were analyzed by statistical software SPSS 19. Results The weight and BMI of the patients in the experimental group was higher than that of control group, the difference was statistically significant(P 〈0.05 ). Body height, systolic blood pressure and diastolic blood pressure of the two groups were not statistically significant ( P 〉 0. 05 ). The Serum HbAlc, FMN in the experimental group were significantly higher than that in the control group, while 1,5-AG levels was significantly lower than that in the control group, the differences were statistically significant ( P 〈 0.05 ). The diagnostic sensitivity of HbAlc, FMN, 1,5-AG and the three combined were 67. 78%, 63. 33%, 83. 33% and 84. 44% respectively. The diagnostic specificity was 94.44%, 93.33%, 90% and 91.11%, respectively. Conclusion HbAlc, FMN and 1, 5-AG can effectively reflect the high blood glucose status of DM. The combined detection of these three can effectively improve the diagnostic sensitivity and specificity of DM, which is worthy further application in order to provide timely diagnosis and proper treatment of DM.
