日常生活能力评定量表在精神科病房分级护理管理中的应用研究

Study on application of activities of daily living scale in grading nursing management of department of psychiatry

目的探析日常生活能力评定量表在精神科病房分级护理管理中的应用效果。方法选取我院精神科病房178例患者作为研究对象,随机分为观察组与对照组各89例;其中对照组由护理人员遵照医生开具的护理级别医嘱进行护理,观察组由护理人员通过观察患者病情,参照日常生活能力评定量表(Activity of daily living scale,ADL)制定护理分级从而进行护理。比较分析两组患者的基础护理质量(皮肤与口腔卫生、衣着卫生、饮食护理、排泄护理、睡眠护理)、护理满意度以及住院期间不良事件发生率。结果观察组的皮肤与口腔卫生、衣着卫生、饮食护理、排泄护理、睡眠护理评分均显著高于对照组(均P<0.001);观察组的护理满意度为96.6%,显著高于对照组85.4%(P<0.001);观察组的不良事件发生率为7.9%,显著低于对照组20.2%(P=0.033)。结论通过结合ADL确定精神科病房患者的护理分级可有效提高基础护理质量、护理满意度,并有效减少患者住院期间的不良事件发生率,同时增加患者的依从性,具有良好的临床实用价值。

Objective To explore application effect of activities of daily living scale in grading nursing manage- ment of department of psychiatry. Methods 178 inpatients treated in department of psychiatry of our hospital from Jan- uary 2013 to March 2016 were selected as the research objects, patients were divided into observation group and control group （ 89 cases in each group ） randomly. The control group was prescribed by the doctor in nursing grades. Patients＇ condition in the observation group were observed by paramedics, ADL Barthel index rating scale （ Barthel Index ） was used as a reference to constitute nursing grade, then nursed the patients. Basic nursing quality （ skin and oral hygiene, clothing hygiene, diet nursing, excretion nursing, sleep nursing ）, nursing satisfaction, incidence of adverse events during hospitalization in the two groups were compared. Results Skin and oral hygiene, clothing hygiene, diet nurs- ing, excretion nursing, sleep nursing scores in the observation group were significantly higher than the control group （all P 〈 0.001 ） . Satisfaction degree of the observation group was 96.6 %, which was significantly higher than the control group （ 85.4 % ）（ P〈0.001 ）. Incidence of adverse events in the observation group was 7.9 %, which was significantly lower than the control group （20.2 % ）（P = 0.033 ）. Conclusion Combination of ADL to confirm nursing grade of psy- chiatric patients can improve the quality of basic nursing, nursing satisfaction effectively, reduce incidence of adverse events of patients during hospitalization, increase the compliance of patients with good clinical practical value.
