瑞舒伐他汀治疗急性肺损伤/急性呼吸窘迫综合征的临床研究

Clinical study of the effect of rosuvastatin on patients with acute lung injury/acute respiratory distress syndrome

目的:探讨瑞舒伐他汀在治疗急性肺损伤/急性呼吸窘迫综合征(ALI/ARDS)中的作用。方法:将60例ALI/ARDS患者在常规治疗基础上随机分配至对照组、瑞舒伐他汀5 mg治疗组、瑞舒伐他汀10 mg治疗组各20例。在治疗前及治疗后14 d分别对患者血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、肌酐(Cr)、肌酸肌酶(CK)、总胆固醇(TC)、总甘油三酯(TG)、C反应蛋白(CRP)、降钙素原(PCT)、氧合指数(OI)、急性生理学和慢性健康状况Ⅱ(APACHEⅡ)评分进行测定、评估比较。结果:与对照组相比,治疗后14 d瑞舒伐他汀治疗组的OI、CRP、PCT及APACHEⅡ评分均显著改善(P<0.05)。与瑞舒伐他汀5 mg治疗组相比,瑞舒伐他汀10 mg治疗组OI、CRP及APACHE改善更为明显(P<0.05),PCT变化虽无统计学意义(P>0.05),但亦显示了改善趋势。治疗后14 d,3组的肝肾功能及CK水平差异无统计学意义(P>0.05)。结论:瑞舒伐他汀对ALI/ARDS具有较好的疗效及安全性。

Objective： To investigate the effect of rosuvastatin on patients with acute lung injury/acute respiratory distress syndrome （ALI/ARDS）. Methods： Sixty patients with ALI/ARDS were divided into three groups randomly, i. e. , control group （ n = 20）, rosuvastatin 5mg group （ n = 20 ） and rosuvastatin 10mg group （ n = 20 ）. Alanine transaminase （ ALT ）, aspartate aminotransferase （ AST）, creatine kinase （ Cr）, total cholesterol （ TC ）, total triglyceride （ TG）, C- reactive protein （ CRP）, procalcitonin （ PCT）, oxygenation index （ OI ） and acute physiology and chronic health evaluations Ⅱ （APACHE Ⅱ） were evaluated before and 14 days after the treatment. Results： Compared with the control group, two rosuvastatin groups showed significant improvements on OI, CRP, PCT and APACHE Ⅱ （P 〈 0. 05 ）. Compared with rosuvastatin 5mg group, rosuvastatin 10 mg group showed significant improvements on OI, CRP and APACHE Ⅱ （ P 〈 0.05）, as well as PCT with an improving trend （ P 〉 0.05 ）. There were no significant differences among the three groups on ALT, AST, Cr and CK （ P 〉 0.05 ）. Conclusion： Rosuvastatin is effective, safe and well tolerated in patients with ALI/ARDS.
