摘要
目的:探讨心力衰竭肺部感染患者血浆脑钠肽与炎性因子的分布情况及其临床意义。方法随机选取心力衰竭合并肺部感染患者47例作为观察组,对其血浆脑钠肽(BNP)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)的水平进行检测记录。另选同期体检的健康人40例作为对照组,对两组受试对象的 BNP 及炎性因子水平进行分析,并探讨上述指标同观察组患者心功能的相关性。结果与对照组相比,观察组血浆 BNP、IL-6及 TNF-α均存在不同程度的上升,差异有统计学意义(P <0.05);其中,随着患者心功能分级的不断上升,其血浆 BNP、IL-6及 TNF-α均表现为持续上升趋势,差异有统计学意义(P <0.05);观察组生化指标与其 NYHA 心功能分级呈正相关(r >0),差异有统计学意义(P <0.05)。结论心力衰竭合并肺部感染患者普遍存在血浆 BNP 及 IL-6、TNF-α等炎性因子水平上升表现,且上述指标与患者的心功能受损程度存在明显相关性,可作为此类患者临床诊断和预后评估的重要依据。
Objective To investigate the distribution and clinical significance of plasma brain natriuretic peptide and inflammatory factors in patients with heart failure and pulmonary infection. Methods Forty - seven heart failure patierrts with pulmonary infection were randomly selected as obser-vation group,and the plasma levels of brain natriuretic peptide(BNP),interleukin-6(IL-6)and tumor necrosis factor( TNF-α)were recorded. Another 40 cases of healthy people were selected as control group,the levels of BNP and inflammatory cytokine of the two groups were analyzed,and the relationship between these indicators and the cardiac function in observation group were investioated. Results Com-pared with the control group,the levels of BNP,IL-6 and TNF-α of observation group increased in vari-erd degrees,the differences were significant(P 〈 0. 05);with the rising of cardiac function classifica-tion,the plasma evels of BNP,IL-6 and TNF-α showed rising trend,there were significant differences (P 〈 0. 05);the biochemical indexes of the observation group were positively correlated with their NYHA cardiac function classification(r 〉 0),the difference was significant(P 〈 0. 05). Conclusions In pa-tients with heart failure and pulmonary infection,the levels of serum BNP,IL-6,TNF-α and other in-flammatory factors increased,and there was significant correlation of the indexes with the degree of cardi-ac dysfunction,the indexes can be used as important basis for clinical diagnosis and prognosis evaluation of such patients.
