超声刀在乳腺癌腋窝淋巴结清扫中的临床应用及效果评价

Clinical application and effect evaluation of ultrasonic knife in axillary lymph node dissection in breast cancer

目的：探讨超声刀进行乳腺癌腋窝淋巴结清扫的临床应用及效果评价。方法以2014年1月至2016年1月行乳腺癌改良根治术患者169例为研究对象，其中75例术中采用超声刀进行腋窝淋巴结清扫（观察组），94例采用电刀清扫腋窝淋巴结（对照组），比较两组手术时间、出血量、引流管留置时间、引流量、淋巴结检出数目和皮下积液的发生情况。结果观察组与对照组相比较，手术完成时间和淋巴结检出数比较差异未见统计学意义（P ＞0.05）；观察组出血量、引流管留置时间、引流量、皮下积液的情况均较对照组减少，差异有统计学意义（P ＜0.05）。结论超声刀在乳腺癌改良根治术中比单用电刀有明显的优势，在乳腺癌根治术中有很好的应用前景。

Objective To investigate the clinical application and effect evaluation of ultrasonic knife for breast cancer axillary lymph node dissection. Methods The 169 cases of breast cancer were en-rolled in this study from January 2014 to January 2016,94 cases were operated by electrosurgical scalpel （observation group）;75 cases were operated by ultracision harmonic scalpel（control group）;Operation time,amount of bleeding during operation,time of drainage tube exelcymosis,amount of drainage ,amount of lymph node,subcutaneous hydrops and complications were observed and recorded accordingly. Results There was no significant difference in operating time or amount of lymph node between the two groups（P 〉0. 05）. Amount of bleeding,the time of drainage tube exelcymosis,amount of drainage and the incidence of subcutaneous hydrops in ultracision harmonic scalpel group were significantly less than those in electrosurgical scalpel group（P 〉0. 05）. Conclusions Harmonic scalpel in modified radical mastectomies have significant advantages,it has a good application prospect in modified radical mastectomies.
