眼眶良恶性肿块表观扩散系数与Ki-67相关性的初步研究 被引量:3

摘要 目的:通过眼眶良恶性肿块表观扩散系数(ADC)值与Ki-67的相关性,探讨ADC值在眼眶良恶性肿块预后的应用价值。方法:对30例眼眶肿块患者(其中良性肿块13例,恶性肿块17例)术前行常规MRI及DWI检查,取2个扩散敏感因子即b值=0和700s/mm^2,测量肿块的ADC值,术后采用免疫组织化学染色方法(SP法)检测肿块标本Ki-67的表达。结果:眼眶良性肿块ADC值分别为(1.38±0.31)×10^(-3)mm^2/s,恶性肿块ADC值分别为(0.80±0.20)×10^(-3)mm^2/s,差异有统计学意义(P<0.01);眼眶肿块的ADC值与Ki-67存在负相关性(r=-0.473,P<0.01)。结论:眼眶肿块ADC值与Ki-67存在一定的相关性,反映细胞增殖程度,为眼眶肿块良恶性及预后提供参考。 Purpose: To assess the value of ADC for evaluating the prognosis of orbital masses through investigating the correlation between apparent diffusion coefficient (ADC) value and Ki-67. Methods: Thirty patients (13 with benign orbital masses and 17 with malignant orbital masses) were enrolled. These patients underwent routine MR and echo-planar DW imaging examinations of the orbits with b values=0 and 700 s/mm^2. The ADC values were observed and compared. The expressions of Ki-67 in orbital masses were detected by immunohistochemical (SP) staining. Results: The mean ADC value of benign orbital masses was (1.38±0.31)× 10^3mm^2/s, while the mean ADC value of malignant orbital masses was (0.08±0.20)× 10^3mm^2/s. There was significant difference between benign and malignant orbital masses (P〈0.01). There was negative correlation between the ADC value and Ki-67. Conclusion: Preliminary results revealed that the ADC values of orbital masses had correlation with Ki-67 and reflected the cell proliferation. It may be applied as a complementary tool in evaluating the prognosis of orbital masses.
