基于中医理论探讨化脓性骨髓炎的病因病机

Discussion the cause of a disease and pathogenesis of suppurative osteomyelitis based on the theory of TCM

文章认为化脓性骨髓炎的病因主要包括外感六淫、邪毒感染、筋骨损伤、七情内伤、正气亏虚、饮食失调、肾虚等;而邪毒乘虚内侵,蕴而化热,导致气血壅塞,化腐成脓蚀骨是该病的基本病机。毒邪聚集于内是病机之关键,正气亏虚是主要病理因素,二者互为因果。肾虚、情志抑郁等是化脓性骨髓炎的重要内在因素。病位主要在筋骨,病理性质为本虚标实,标实为主。扶正祛邪是其基本治疗大法,贯穿治疗的始终。

The chief cause of suppurative osteomyelitis included six climate exopathogens,infection of pathogenic toxin,injuries of bones and muscles,internal damage by excess of seven emotions,deficiency of vital qi,improper diet,deficiency of kidney,etc;Pathogenic toxin invading people in the weakness of bodies,accumulating and heating,obstructing the way of qi and blood,resulting in the formation of abscess and erosion of bones which are the basic pathogenesis of suppurative osteomyelitis.Among which the accumulation of pathogenic toxin is the key point of the pathogenesis,and the deficiency of vital qi is the main pathological factors,the both are cause and effect for each other.The deficiency of the kidney,over-emotional depression are the internal factors of the suppurative osteomyelitis.The location of disease concerns the muscles and bones mainly,the nature of disease is deficiency in the ＇root＇ and excess in the ＇branch＇,the excess in the ＇branch＇ is the main aspect.The therapeutic rule of the disease is strengthening the body resistance and eliminating pathogenic factors,which should be applied from the beginning to the end.
