住院患者预防跌倒信息化安全管理模式的建立与实施

Establishment and practice of an informational safety management model to prevent inpatient falls

目的建立住院患者预防跌倒信息化安全管理模式,降低住院患者跌倒发生率。方法建立住院患者预防跌倒信息化安全管理模式,主要措施包括建立跌倒风险评估及风险预警平台,制订预防跌倒集束化管理策略,制作预防跌倒电子宣教素材,开展基于网络平台的预防跌倒规范化培训,构建跌倒事件上报与追踪平台等;在全院106个护理病区实施,比较实施信息化安全管理前后的效果。结果实施预防跌倒信息化安全管理模式后,住院患者跌倒发生率显著降低(P<0.01),预防跌倒措施指标落实率显著提升(均P<0.01)。结论预防跌倒信息化安全管理模式的建立与实施,有利于过程控制跌倒高风险因素,实现住院患者预防跌倒高效管理,降低住院患者跌倒发生率,保障患者安全。

Objective To establish an informational safety management model to prevent inpatient falls, and to reduce the rate of in- patient falls. Methods An informational safety management model was established to prevent inpatient falls, the main interventions of which included establishing a falls risk assessment and risk early warning platform, developing a bundle of management strategy to prevent falls, making electronic propaganda and education material of falls prevention, conducting falls prevention standardized training based on network platform and building an incident reporting and tracking platform. Then the model was implemented in 106 units of the hospital, and the effects before and after the informational safety management were compared. Results After imple mentation of the informational safety management model, the rate of inpatient falls decreased significantly （P〈0.01）, and the in dex implementation rate on falls prevention intervention increased significantly （P〈0.01 for all）. Conclusion Implementation and practice of the informational safety management model to prevent inpatient falls is conducive to control high risk factors of falls, achieve high efficacy management of inpatient falls prevention and reduce the rate of inpatient falls, then guarantee their safety.
