细胞炎性因子诊断急性上呼吸道感染的临床应用价值研究

目的探讨血清降钙素原(PCT)、C-反应蛋白(CRP)及白细胞介素-18(IL-18)在急性上呼吸道感染(AURI)中的临床诊断价值,为临床诊断提供参考。方法将本院收治的AURI患者共97例作为观察组,同时选择同期体检健康者46例作为对照组,观察两组的血清PCT、CRP及IL-18水平,检测阳性率;细菌感染组在治疗前后血清PCT、CRP及IL-18的含量水平变化,研究所得数据采用SPSS19.0统计学软件给予处理。结果血清PCT、CRP、IL-8比较:对照组与病毒组之间无明显差异,病毒组和对照组低于细菌组(P<0.05);PCT诊断特异性和敏感性最好;观察组各炎症指标阳性率均高于对照组(P<0.05);细菌感染组在使用抗菌药物治疗后1 d、3 d、7 d,血清PCT、CRP及IL-18含量较治疗前均有所下降(P<0.05),且在第7天各指标的含量处于最低。结论急性上呼吸道感染患者血清PCT、CRP及IL-18水平均明显升高,其中血清PCT诊断细菌性急性上呼吸道感染的敏感性和特异性最高,有临床推广价值。

Objective To investigate the clinical value of serum CRP （PCT）, reactive protein （C-） and interleukin -18 （IL-18） in patients with acute upper respiratory tract infection（AURI）, and to provide reference for clinical diagnosis.Methods 97 cases of AURI treated in our hospital as the observation group, 46 cases were selected as the control group.The serum levels of PCT, CRP and IL-18 were observed in the two groups, and the positive rate was detected;The content of PCT, CRP and IL-18 in the sera of pa- tients with bacterial infection before and after treatment,data from this study dealt with using SPSS19.0 statistical software.IRe- suits Comparison of serum PCT, CRP and IL-8： there was no significant difference between the control group and the virus group, the virus group and the control group were lower than the bacterial group （P〈0.05）; the PCT diagnostic specificity and sensitivity were the best;The positive rate of inflammation index in observation group was higher than that in control group （P〈0.05）;Bacterial infection after antibiotic treatment the lth day, 3rd day, 7th day, PCT, levels of CRP and IL-18 were compared with before treatment significantly decreased （P〈0.05）, 7 d index declined to the lowest levels.Conclusion In patients with acute upper respiratory tract infection, the serum levels of PCT, CRP and IL- 18 were significantly increased,The sensitivity and specificity of serum PCT in the diagnosis of bacterial acute upper respiratory tract infection was the highest, and it was worth to be popularized in clinic.
