Quadrant系统下经伤椎置钉单节段治疗胸腰椎骨折的临床研究

Clinical study on the treatment of thoracolumbar vertebral fracture with single segment of vertebral pedicle screw under Quadrant system

目的探讨Quadrant系统下经伤椎置钉单节段治疗胸腰椎骨折的临床效果。方法将80例胸腰椎骨折患者采用随机数字表法分为对照组及研究组各40例。对照组采取开放椎弓根钉内固定治疗,研究组采取Quadrant系统下经伤椎置钉单节段治疗,比较两组患者临床治疗效果。结果研究组并发症发生率10.0%(4/40),显著低于对照组的20.0%(8/40),两组比较,差异有统计学意义(P<0.05);围术期相关指标、椎间活动度、术后影像学参数和VAS评分相比较,研究组优于对照组,两组比较,差异有统计学意义(P<0.05)。结论 Quadrant系统下经伤椎置钉单节段治疗胸腰椎骨折取得的效果相较于开放椎弓根钉内固定治疗更佳,可作为临床治疗的优选方案加以推广使用。

Objective To explore the clinical effect of single segment of vertebral pedicle screw under Quadrant system in the treatment of thoracolumbar vertebral fracture. Methods 80 patients with thoracolumbar vertebral fracture were divided into control group and study group according to the random number table,40 cases in each group. The patients in the control group were treated with open pedicle screw internal fixation,while the patients in the study group were treated with percutaneous pediculoplasty with Quadrant system. The clinical effect between the two groups were compared. Results The incidence rate of complications in the study group was 10%（ 4 /40）,significantly lower than 20. 0% in the control group（ 8 /40）,the difference was statistically significant（ P〈0. 05）; Compared with the control group,the indexes of the perioperative period, the degree of activity of the intervertebral disc, the postoperative imaging parameters and the VAS scores of the study group were better than those of the control group,the difference was statistically significant between（ P〈0. 05）. Conclusion The clinical effect of percutaneous pediculoplasty under Quadrant system is better than open pedicle screw internal fixation in the treatment of thoracolumbar vertebral fracture,which can be used as an optimization scheme in clinic.
