超声造影在细小乳腺肿物微创治疗中的指导意义

Significance of contrast enhanced ultrasound in minimally invasive treatment of small breast tumors

目的:探讨超声造影细小乳腺肿块微创治疗的指导意义。方法:将B超随诊发现且触诊不明显的细小乳腺肿物152例患者分为对照组和试验组,对照组采用普通B超检查,试验组除普通B超检查外加超声造影检查,两组患者均实施手术治疗,结合术后病理评价超声造影在治疗此类患者中的指导意义。结果:所有肿块均成功旋切,每枚肿块平均手术时间(14.0±6.3)min,平均旋切(11.0±5.7)次;所有肿块术后均行常规病理检查,152例患者中良性病变132例,恶性肿瘤20例,其中Bi-RADS分级2级患者共72例,病理结果均为良性;Bi-RADS分级3级患者共38例,对照组良性患者17例,恶性患者4例,试验组良性患者16例,恶性患者1例;Bi-RADS分级4级患者共36例,对照组良性患者9例,恶性患者5例,试验组良性患者18例,恶性患者3例;Bi-RADS分级5级患者共9例,对照组良性患者1例,恶性患者2例,试验组良性患者1例,恶性患者5例。结论:超声造影对诊断早期恶变的微小乳腺肿瘤有一定指导意义,术前行常规行造影评估,有助于乳腺恶性肿瘤的早期诊断、早期治疗。

Objective： To study the significance of contrast enhanced ultrasound in minimally invasive treatment of small breast tumors. Method： 152 patients with insignificant small breast lesions found in ultrasound follow-up and palpation were divided into the control group and the experimental group, the control group received common ultrasonic examination while the experimental group received contrast-enhanced ultrasound examination in addition to the ordinary ultrasound and both groups of patients received surgery treatment, postoperative pathological evaluation of contrast-enhanced ultrasound was made after the treatment of these patients. Results： All lumps are successful rotary cutting, the average operation time of each mass was （14.0±6.3） min, the average rotary cutting was （11.0 ± 5.7） times; All masses were given postoperative routine pathological examination, 132 cases were with fibroadenoma, and 20 cases were carcinoma. 72 cases were Bi-RADS grade 2 patients, of whom the pathological findings were benign; 38 cases were Bi-RADS grade 3 patients, 17 cases were benign and 4 cases were malignant in the control group while 16 cases were benign and 1 case was malignant in the experimental group; 36 cases were Bi-RADS grade 4 patients, 9 cases were benign and 5 cases were malignant in the control group while 18 cases were benign and 3 cases were malignant in the experimental group; 9 cases were Bi-RADS grade 5 patients, 1 case was benign and 2 cases were malignant in the control group while 1 case was benign and 5 cases were malignant in the experimental group. Conclusions： Contrast enhanced ultrasonography had certain guiding significance in the early diagnosis of malignant breast tumor, preoperative conventional imaging evaluation helped to improve the early diagnosis and treatment of breast malignant tumors.
