联合检测LDL-VLDL—GGT9／6、GGT活性在冠心病病变严重程度评估中的应用

目的回顾性分析冠心病病变严重程度与血清低密度脂蛋白胆固醇（LDL）、r-谷氨酰氨基转移酶（GGT）、LDL-VLDL_GGT、LDL，VLDL—GGT％相关指标之间的关系。方法测定465例不稳定心绞痛（UA），665例稳定性心绞痛（SAP），126例造影结果阴性的对照者。均相酶比色法测定LDL、速率法测定GGT和LDL-VLDL-GGT、计算LDL-VLDL—GGT占GGT总活性的百分比。比较上述三组间的差异。所有造影结果阳性患者随访一年，并记录再次运行心脏事件（包括心肌梗死、死亡），分析血清LDL、GGT、LDL—VLDL_GGT、LDL—VLDL—GGT％与再次运行心脏事件之间的关系。结果uA组GGT总活性（43．74±18．78U／L）显著高于SAP组（30．13±11．43U／L，P〈0．05）UA组LDL-VLDL—GGT％（23．28±12．24）与SAP组（8．25±3．34）比较，差异显著（P〈O．01），而LDL-VLDL—GGT（34．14土18．39U／L）与sAP组（25．13±11．17U／L）比较差异不显著，（P〉0．05）；LDL在uA组（5．42±2．56mmol／L）与SAP组（5．39±2．43mmol／L）比较差异不显著（P〉0．05）。SAP组血清LDL、GGT、LDL—vLDL_GGT、LDL—VLDL—GGTO明显高于阴性对照组（P〉0．05）。跟踪随访一年，高浓度GGT（〉60．56U／L）、LDL—VLDL-GGT％（〉30．26）的患者再次心脏事件的发生率较高。结论联合检测LDL、GGT、LDL—VLDL-GGT、LDL—VLDL-GGT％可更好了解冠心痛病变程度，与冠心病病变程度密切相关，随访显示血清GGT总活性、LDLVLDL-GGT％高的患者心脏事件发生率较高。

Objective To retrospective analysis the relationship between the severity of coronary heart disease and LDL,total GGT activity,LDL-VLDL-GGT and LDL-VLDL-GGT%. Methods unstable angina group （n=465）,stable angina peetoris group （n= 665） and the control group （n= 126）. The level of serum LDL was measured by Homogeneous enzyme colorimetric method, the level of serum total GGT activity Was measured by enzyme method, the level of serum LDL-VLDL-GGT and LDL-VLDL- GGT% was also measured. Compare the differences between the three groups. The efficacy of therapy and cardiovascular events ineluding myocardial infarction revascularization or death were recorded follow-up of one year. Results The levels of total GGT activ- ity（43. 74±18.78U/L）,LDL-VLDL-GGT（23. 74±12.78）in UA group were significantly higher than SAP group （30. 13±11. 43U/L; 8. 13±3.43） ,with P〈0.05. The levels of FG, HS-CRP in patients of UA group and the SAP group were not significantly different. The levels of LDL, total GGT activity, LDL-VLDL-GGT, LDL-VLDL-GGT% in SAP Group was significantly higher than the control group,with P all 〈0.05. The rate of subsequent cardiovascular events is higher in the patients with elevated level of total GGT activity 〉60. 56U/L, LDL-VLDL-GGT%〉30. 26. Conclusion LDL, The total GGT activity, LDL-VLDL-GGT, and LDL-VLDL-GGT% are closed with the severity of coronary heart disease, especially in total GGT activity and LDL-VLDL-GGT%. The total GGT activity and LDL-VLDL-GGT% are very closed with the severity of coronary heart disease. The levels of total GGT activity and LDL-VLDL-GGT show that the recurrence rate of cardiovascular events is higher in the patients.
