γ-干扰素体外释放酶联免疫定量试验在肺结核病诊断中的应用 被引量:7

摘要 目的探讨结核杆菌γ-干扰素体外释放酶联免疫定量试验(TB-IGRA)在肺结核病诊断的应用价值。方法收集肺结核组(43例)、非肺结核组(38例)及健康对照组(30例)全血、血清及痰液进行TG-IGRA、结核抗体、抗酸染色、痰TB-DNA检测,分析TBIGRA敏感性、特异性等诊断方法学指标,并且比较各种诊断方法的敏感性。结果TBIGRA敏感性为81.4%(35/43),特异性为91.2%(62/68),阳性预测值为85.4%(35/41),阴性预测值为88.6%(62/70),抗酸染色、结核抗体、TB-DNA敏感性分别为48.8%,44.2%,65.1%,IGRA和其他3种试验敏感性差异有统计学意义(P=0.002)。43例肺结核患者中,徐阳组和冷阴组IGRA阳性检出率分别为90.5%,72.7%(P=0.240),差异无统计学意义。结论TB-IGRA在四种方法中敏感性和准确度最高,具有较高的特异性,对肺结核病有一定的诊断价值。该方法操作简单,适宜在基层医院推广使用。
