超声三维斑点技术对心肌梗死患者心功能的评价作用研究 被引量:4

摘要 目的探讨超声三维斑点技术对心肌梗死患者心功能的评价作用。方法选择2015年2月至2016年2月诊治的心肌梗死患者40例为观察组,同期40例健康志愿者为对照组。两组均接受超声检查,观察对象心功能指标的变化;分析心肌梗死患者三维斑点超声测量结果与单光子发射计算机化断层显像(SPECT)的相关性。结果与对照组比较,观察组左室舒张末期容积(LVEDV)、左室每搏输出量(SV)及左室射血分数(LVEF)均显著降低,舒张末期室间隔厚度(IVST)及舒张末期左室后壁厚度(LVPWT)均显著升高,差异具有统计学意义(P<0.05)。在观察组内,超声三维斑点技术与SPECT对心肌梗死患者LVEDV、LVESV及LVEF测量值比较,差异无统计学意义(P>0.05)。以SPECT为金标准,心肌梗死患者三维斑点超声测量结果 LVEDV、LVESV及LVEF的测量值与SPECT具有显著的正相关(r=0.969,0.958,0.942,P<0.05)。结论超声三维斑点技术可以准确地评价心肌梗死患者的心功能。 Objective To investigate the effect of three dimensional speckle technique on cardiac function in patients with myocardial infarction. Methods From February 2015 to February 2016,40 patients with myocardial infarction were selected as the observation group. At the same time,40 healthy volunteers were served as control group. Ultrasound examination were performed to observe the changes of cardiac function indexes in patients with myocardial infarction. The correlation between the results of three dimensional speckle ultrasound measurement and SPECT in patients with myocardial infarction was analyzed. Results Compared with the control group,observation group,left ventricular diastolic end diastolic volume( LVEDV),LVESV,left ventricular stroke volume( SV) and left ventricular ejection fraction( LVEF) were significantly decreased. End diastolic ventricular septal thickness( IVST) and end diastolic left ventricular posterior wall thickness( LVPWT) were significantly increased. The difference was statistically significant(P〈0. 05). In the observation group,the difference of LVEDV,LVESV and LVEF in patients with myocardial infarction between three- dimensional speckle technique and SPECT was not statistically significant(P〈0. 05). With SPECT as the gold standard,LVESV and LVEF have significant positive correlation with LVEDV( r = 0. 969,0. 958,0. 942,P〈0. 05) in the three dimensional speckle ultrasound measurement results in myocardial infarction patients. Conclusion Three dimensional speckle technique can accurately evaluate the cardiac function in patients with myocardial infarction.
