胸腔积液ADA、IFN-γ、TNF-α与胸腔积液沉渣DNA联合检测在结核性胸膜炎诊断中的价值

Value of combined examination of ADA,IFN-γ,TNF-αand pleural effusion sediment DNA for diagnosis of tuberculous pleuritis

目的探讨胸腔积液腺苷脱氨酶(ADA)、γ-干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)与胸腔积液沉渣DNA(TBDNA)联合检测在结核性胸膜炎诊断中的价值。方法选取2014年5月至2015年11月经诊断为结核性胸腔积液的62例患者作为结核性胸腔积液组,另选取62例非结核性胸腔积液的患者作为对照(非结核组为恶性胸腔积液),采用酶比色法检测ADA活性,采用酶联免疫吸附试验(ELISA)检测IFN-γ和TNF-α,采用PCR定性检测TB-DNA。计算4项指标单独检测和联合检测结核性胸腔积液的敏感性和特异性。结果结核性胸腔积液组的ADA、IFN-γ和TNF-α水平均明显高于非结核性胸腔积液组(P<0.05)。ADA、IFN-γ、TNF-α和TB-DNA对结核性胸腔积液组的单独检测阳性率均明显高于非结核性胸腔积液组的检测阳性率(P<0.05)。4项指标联合检测结核性胸腔积液组的阳性率显著高于非结核性胸腔积液组(P<0.05)。4项指标联合诊断结核性胸膜炎的敏感性和特异性分别为91.9%和93.5%,均显著高于各指标单独诊断的敏感性和特异性。结论联合检测胸腔积液中ADA、IFN-γ、TNF-α和TB-DNA能够提高诊断的准确性,可以作为早期诊断与鉴别结核性胸膜炎的重要参考指标。
