摘要
目的观察安珂(ENCOR)真空辅助旋切系统在乳腺微创外科中的临床应用价值。方法选取医院收治的乳腺肿瘤患者260例,根据治疗方法不同分为观察组(180例)和对照组(80例)。观察组采用安珂真空辅助旋切系统切除治疗,对照组采用常规乳腺肿瘤切除术治疗,对于一些临床不能触及的病灶,采用术前超声引导下导丝定位。观察2组手术时间、术中出血量、瘢痕长度等手术情况及术后并发症发生情况,比较2组随访肿瘤残留、复发率。结果观察组手术时间、术中出血量、瘢痕长度均优于对照组(P<0.05或P<0.01);观察组术后并发症发生率为6.1%,低于对照组的21.3%(P<0.05)。随访结果,2组肿瘤残留比例、复发率比较差异均无统计学意义(P>0.05)。结论安珂(ENCOR)真空辅助乳腺微创旋切系统切除乳腺肿瘤并发症少,美容效果佳,尤其对于临床不可触及的微小病灶、位于不同象限的多个肿瘤的处理,优势明显,是临床上值得推广的手术方式。
Objective To study the clinical application value of ENCOR vacuum auxiliary rotary system in breast minimally invasive surgery. Methods 260 patients with breast cancer were selected and divided into observation group ( n = 180) and control group (n = 80) according to different treatment methods. The observation group received ENCOR vacuum assisted biopsy system in resection, while the control group was treated with conventional breast tumor resection. For some of the clinical lesions that can not be touched, we use ultrasound guided wire localization before surgery. The operative time, intraoperative blood loss, the length of scar, and postoperative complications were observed in 2 groups. The residual and recurrence rates of the 2 groups were compared. Results The operation time, blood loss and scar length in the observation group were bet- ter than those in the control group ( P 〈 0.05 or P 〈 0.01 ). The complication rate was 6.1% patients in the observation group, which was less than 21.3% in the control group(P 〈 0.05 ). There was no significant difference between the 2 groups in tumor residual rate and recurrence rate(P 〉 0.05 ). Conclusion ENCOR vacuum assisted minimally invasive breast rotary cutting system to remove breast tumor has fewer complications, and better cosmetic effect. Especially for the non palpable small lesions, located in different quadrants of the processing of multiple tumors, it has obvious advantages ,which is worthy of clinical promotion mode of operation.
