我院时间依赖性抗菌药物输注间隔时间不规范的原因及对策分析

Analysis of Causes and Countermeasures for Nonstandard Infusion Interval Time of Time-dependent Antibiotics in Our Hospital

目的:为规范时间依赖性抗菌药物输注间隔时间提供参考。方法:采用横断面调查研究,随机选择2016年5月28日我院使用时间依赖性抗菌药物的住院患者336例,对其时间依赖性抗菌药物输注间隔时间进行调查和分析,发现存在的问题,分析可能的原因并提出改进措施。结果:我院使用的时间依赖性抗菌药物大部分为β-内酰胺类药物;bid给药的时间依赖性抗菌药物输注间隔时间<8 h的占84.62%;不规范间隔用药在bid给药与定点给药(q8 h或q12 h)的患者之间(68.75%vs.4.17%),差异有统计学意义(P<0.05);不同科室和病区不规范输注间隔的发生率有较大差别。分析原因可能与医师、护士、科室及其他医疗条件等因素有关。结论:我院住院患者时间依赖性抗菌药物输注间隔时间过短,不规范情况突出,应从医师、护理人员、科室等多方加强管理、教育与培训,以保障患者用药安全、有效。

OBJECTIVE： To provide reference for standardize infusion interval time of time-dependent antibiotics. METHODS： In cross-sectional study, 336 inpatients receiving time-dependent antibiotics were randomly selected from our hospital in May 28th, 2015. The infusion interval of time-dependent antibiotics was investigated and analyzed to find out the problems, analyze the causes and put forward the improvement measures. RESULTS： Most of the time-dependent antibiotics used in our hospital are β-lactam antibiotics. 84.62% time-dependent antibiotics of the bid infusion interval was lower than 8 h; the proportion of patients with nonstandard infusion interval of bid and regalar use （q8 h or q12 h）（68.75 % vs. 4.17 % ）, with statistical significance （P〈 0.05）. There was great difference in the incidence of nonstandard infusion interval among different departments and wards. It may be associated with physicians, nurses, departments and other medical conditions. CONCLUSIONS： The infusion interval time of time-dependent antibiotics is too short in the inpatients of our hospital, and nonstandard infusion interval is serious; it is suggested to strengthen management, education and training from physicians, nurses and departments, so as to guarantee the safety and effectiveness of drug use.
