摘要
《临证指南医案》中的血证医案(卷二吐血、卷七便血、卷八衄)体现了叶天士诊疗血证的学术思想,结合各家经验,对《临证指南医案》中诊疗血证的病案及方药进行归纳、分析,认为叶天士在《临证指南医案》中诊疗血证多从脾胃着手,脾胃分治,重视胃阴,以甘寒之品滋养胃中阴液;不忽视脾阳,用燥湿之药理脾,顾护脾胃的同时亦不闭门留寇。脾胃同治,以甘淡薄味养胃补脾,助脾胃升清降浊,恢复中焦受纳运化之权。
The blood syndrome records （hematemesis, hematochezia, hemorrhinia） shows the YE tian-shi＇s treating experi- ence in blood syndrome in the Clinic Guidline of Medical Records. The author analyzed medical records and herbs, based on other medical practitioners＇s treating experience of the blood syndrome, then summarized main characteristics. The treatment of blood syndrome ought to completely concern on the spleen and stomach which are treated separately. The medical practitioners paid attention to gastric Yin, used sweet-cold herbs nourishing gastric Yin without ignoring splenic Yang. Also, it utilized dry-wet herbs to nourish spleen, protect spleen and stomach, and do not leave the evil in. It helps spleen and stomach shengqingjiangzhuo and recovers middle- jiao~ right of digestion and absorption by treating spleen and stomach together and utilizing sweet-thin herbs nourishing spleen and stomach.
