急性脑卒中患者血清瘦素朋旨联素比值与认知损害的关系

The correlation between leptin to adiponectin ratio in serum and cognitive impairment of acute stroke patients

目的探讨急性脑卒中患者血清瘦素朋旨联素比值（L/A）与认知功能损害的关系。方法收集2014—09～2015—09急性脑卒中患者共计128例，检测发病时血清L/A，于3个月后根据测量MMSE、MoCA等评分及相应标准，将患者分为VaD组（n=24）、VCI组（n=37）及对照组（n=67）。结果VaD组患者血清L／A比值高于VCI组，且两组均高于对照组（2．77vs．1．33，P〈0．05）；血清L／A比值与MMSE评分呈负相关（r=-0．464，P〈0．05），与MoCA评分呈负相关（r=-0．519，P〈0．05）；ROC曲线面积为0．771，诊断准确性相对较高，当L／A比值为1．76时，对应的敏感度和特异度之和（约登指数）最大。结论急性脑卒中患者血清L／A比值与认知功能损害程度呈正相关，L／A比值对急性脑卒中后认知功能障碍的诊断有一定的提示意义。

Objective To explore the correlation between leptin to adiponectin ratio （L/A ratio） in serum and cognitive impairment of acute stroke patients. Methods We collected 128 acute stroke patients during September 2014 to September 2015. We examined the serum levels of L/A ratio when they suffered from acute stroke, and divided the patients into VaD group （n = 24）, VCI group （n = 37） and control group （n = 67） according to MMSE scores, MoCA scores and the criteria we used after 3 months. Results The L/A ratio of VaD group was higher than that of VCI group, and both were higher than the ratio of control group. The difference was statistical significant （ P 〈 0.05）. The IdA ratio was negatively related to MMSE scores （r =-0.464, P 〈 0.05）, and MoCA scores （r =-0.519, P 〈 0.05）. In addition, the area under curve of ROC was 0.771, which indicated relatively high diagnosis accuracy. The best cut- off level of IdA ratio was 1.76. Conclusion The serum IdA ratio of acute stroke patients was positively related to severity of cognitive impairment, and might be helpful for the diagnosis of post-stroke cognitive impairment.
