外科药房口服药物不合理医嘱调查分析

Analysis of unreasonable medical orders on oral medicine in surgery pharmacy

目的对外科药房口服药物不合理医嘱进行分析,了解不合理用药情况,提高用药安全性、合理性、有效性。方法对外科药房2015年全部口服药物不合理医嘱常见问题进行审核、统计、分类和分析。结果不合理医嘱1 472条,主要不合理医嘱问题集中在给药频次、给药剂量、重复用药、联合用药、给药途径不适宜等问题。结论住院药师应立足于药房,为临床提供更加可靠的用药信息,通过医嘱审查联络信方式与临床科室沟通,为保障患者安全、合理、有效用药做出贡献。

Objective To analyze the unreasonable medical orders on oral medicine in surgery pharmacy,in order to learn about the status of irrational drug use and improve the safety,rationality and efficacy.Methods All the unreasonable prescriptions of oral medicine in surgical pharmacy in 2013 were collected,and the inspection,statistics,classification and analysis for the common problems were performed.Results There were 1 472 unreasonable prescriptions in total,and the questions focused on improper frequency,dosage,repeated use drug combination and administration ways and so on.Conclusion Through communicating with the doctors of clinical departments by way of writing letters,hospital pharmacists should provide more reliable drug information for clinical medication in order to contribute to the safe and rational drug use for patients.
