Detection of cancer therapy-related cardiac injure

摘要 随着医疗技术的进步,肿瘤患者的生存时间明显延长,但肿瘤治疗(包括化疗及放疗)相关心脏损伤的发生率及致死率也不断增加。及时对肿瘤患者进行肿瘤治疗相关心脏损伤的检测有利于指导临床中的诊疗策略,进而改善患者的预后。文章根据相关研究及最新的2016年ESC指南,总结了目前较为常用的检测手段包括心电图、超声心动图(可测量左心室射血分数、整体纵向应变)及一些生物标志物(肌钙蛋白、高敏肌钙蛋白、B型脑钠肽、N末端B型脑钠肽前体)的检测等。同时阐述相关检测指标在评价肿瘤治疗相关心脏损伤方面的应用价值。 Advances in medical technology have resulted in significant improvement in long-term survival for many cancer patients, but have also resulted in untoward side effects. Cardiac injure is one of the most concerning side effects of cancer therapy. Early detection of cancer therapy-related cardiac injure is helpful to guide the diagnosis and treatment strategies, and improve the survival of cancer patients. This review will summarize some common detecting techniques including electrocardiogram, echocardiography (such as LVEF, GLS) and some biomarkers (TnI, hs-TnI, BNP, NT-proBNP), evaluate the significance of these parameters in detecting the cardiac injure of cancer treatment according to the related research and 2016 ESC guidelines.
