OLF方案与DCF方案在基层医院治疗晚期胃癌疗效比较

A clinical study of OLF regimen compared with DCF regimen in treatment of advanced gastric cancer in basic hospitals

目的:探究基层医院OLF方案对比DCF方案治疗晚期胃癌的临床疗效及其安全性。方法:收集2009年1月至2012年12月期间在本院接受胃癌根治术(D2)、病理组织学证实为进展期胃癌的患者资料,选取其中60例接受术后OLF方案(奥沙利铂+亚叶酸钙+5-FU)化疗的患者作为研究组,另选取60例同期接受DCF方案(多西紫杉醇+顺铂+5-Fu)的胃癌患者作为对照组,比较两组临床疗效及安全性。结果:研究组患者全部可评价疗效,完全缓解(CR)6例,部分缓解(PR)30例,总有效率(CR+PR)为60.00%,对照组患者全部可评价疗效,CR 1例,PR 24例,有效率41.67%,两组患者临床有效率差异有统计学意义(P<0.01)。研究组患者白细胞减少17例,恶心呕吐27例,对照组患者白细胞减少48例,恶心呕吐39例,研究组不良反应发生率明显低于对照组(P<0.05)。结论:OLF方案和DCF方案均是基层医院治疗晚期胃癌可行的化疗方案。OLF方案疗效更优,耐受性更好,是胃癌辅助化疗的较好方案。

Objective Toexplore the efficacy and safety of OLF regimen versus DCF regimen in treatment of advanced gastric cancer in basic hospitals. Methods Clinical data on patients with advanced gastric cancer confirmed by histopathology after gastrectomy （D2） during January 2009 and December 2012 were collected. 60 patients receiving post-operative OLF regimen chemotherapy were assigned to a study group, and 60 patients on DCF regimen chemotherapy during the same period were assigned to a control group. The clinical efficacy and safety in the two groups were compared. ResultsIn the study group, all the patients were evaluated. 6 patients received complete remission （CR） and 30 achieved partial remission （PR）, and the response rate （CR ＋ PR） was 60.00%. All the patients were evaluated in the control group. One patient obtained CR and 24 received PR, and the response rate was 41.67%. The response rate between the two groups were statistically different （P 〈 0.01）. In the study group, 17 patients developed leukopenia and 27 developed nausea and vomiting; while in the control group, 48 patients developed leueopenia and 39 developed nausea and vomiting. The incidence of adverse reactions was significantly lower in the study group than in the control group （P 〈 0.05）. Conclusions BothOLF and DCF chemotherapy are the feasibleregimens for the treatment of advanced gastric cancer in basic hospitals. OLF is more effective and tolerable, and it is a better regimen in adjuvant chemotherapy.
