摘要
目的研究白介素-6(IL-6)基因启动子区-572C/G位点的基因多态性与急性脑梗死患者继发肺部感染风险之间的相关性。方法应用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)法,测定2010年1月-2015年12月435例脑梗死住院患者IL-6基因启动子区-572C/G位点的基因多态性,根据不同的基因型分为CC组和CG+GG组,比较两组间的一般情况以及继发肺部感染情况。结果两组脑梗死患者的性别、年龄、糖尿病史、高血压史、吸烟史、饮酒史、体质量指数(BMI)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL)、NIHSS评分等比较差异均无统计学意义(P>0.05);CC组出现肺部感染共13例,占5.75%,而CG+GG组出现肺部感染共24例,占11.48%,两组间比较差异有统计学意义(P=0.032),其中CG+GG组的肺部感染率明显高于CC组。结论 IL-6基因-572C/G多态性可能与急性脑梗死患者继发肺部感染的风险有关,其中CG及GG基因型是急性脑梗死患者继发肺部感染的易感基因型,而CC基因型可能为保护基因型;G基因可能是易感基因,而C基因可能为保护基因。
OBJECTIVE To study the correlation between the gene polymorphism of interleukin-6(IL-6) gene pro- moter region -572 C/G locus and the risk of secondary pulmonary infections in patients with acute cerebral infarc- tion. METHODS A total of 435 patients with acute eerebral infarction who were hospitalized from Jan 2010 to Dec 2015 were enrolled in the study, then the gene polymorphism of IL-6 gene promoter region -572 C/G locus of the patients was determined by using polymerase-chain-reaction-restriction fragment length polymorphism (PCR- RFLP) method, all specimens were divided into the CC group and the CG+GG group according to the genotypes, and the baseline data and ineidence of secondary pulmonary infection were observed and compared between the two groups. RESULTS There was no significant difference in the genders, age, history of diabetic mellitus, history of high blood pressure, smoking history, drinking history, body mass index (BMI), triglyceride (TG), low density lipoprotein cholesterin (LDL), or NIHSS between the two groups of cerebral infarction patients (P〉0.05). Totally 13 patients had pulmonary infection in the CC group, accounting for 5.75%, while 24 patients had pulmonary infection in the CG+ GG group, accounting for 11.48%, and there was significant difference between the two groups (P= 0. 032); the incidence rate of pulmonary infection was significantly higher in the CG+GG group. CONCLUSION The IL-6 gene -572 C/G polymorphism may be associated with the risk of secondary pulmonary infect/on in the patients with acute cerebral infarction, the acute eerebral infarction patients who have the CG and GG genotypes are at high risk of the secondary pulmonary infections, however, the CC genotype may be the protective genotype, the G gene may be a susceptible gene, and the C gene may be a protective gene.
