三维超声诊断胎儿肢体畸形的临床应用意义

摘要 目的探讨三维超声诊断胎儿肢体畸形的临床应用意义。方法选取我院2008年1月至2014年8月进行超声检查的孕妇。所有孕妇经过二维超声检查后,怀疑存在胎儿肢体发育异常现象55例,然后有针对性地进行三维超声检查,并将二维超声检查和三维超声检查与产后结果进行对比。结果采用二维超声诊断胎儿的敏感度、特异度和准确度分别为69.05%、46.15%和63.64%,采用三维超声诊断的敏感度、特异度和准确度分别为90.48%、84.62%和89.09%,差异有统计学意义(P<0.05)。55例胎儿中有42例肢体畸形,其畸形种类共13种。结论采取三维超声诊断能够提高胎儿肢体畸形临床诊断的敏感度及准确度,而且能够准确地反映胎儿肢体结构和畸形状况,有利于为临床中制定合理处理方案提供重要参考依据。 Objective To explore the clinical significance of three-dimensional ultrasonography in diagnosis of fetal limb deformity.Methods The pregnant women undergoing ultrasound examination in our hospital from January 2008 to August 2014 were selected. Aftertwo-dimensional ultrasonography, 55 cases suspected with fetal dysmelia received targeted three-dimensional ultrasonography. Thepostpartum results were compared with those of two-dimensional ultrasonography and three-dimensional ultrasonography. Results Thesensitivity, specificity and accuracy of two-dimensional ultrasonography were respectively 69.05%, 46.15% and 63.64%, while those ofthree-dimensional ultrasonography were respectively 90.48%, 84.62% and 89.09%, with statistical difference (P 〈0.05). 42 cases of fetallimb deformity was found in 55 cases, with a total of 13 kinds of deformity. Conclusions Three-dimensional ultrasonography can improvethe sensitivity and accuracy of clinical diagnosis of fetal limb deformity, accurately reflect the fetal limb structure and abnormal conditions,which helps to provide important reference for reasonable treatment scheme in clinic.
