乙肝肝硬化合并自发性细菌性腹膜炎患者血清内毒素、降钙素原水平及其临床价值

The Levels of LPS and PCT of the Patients with HBV-liver Cirrhosis Complicated with SBP and the Clinical Value

目的调查内毒素(LPS)、降钙素原(PCT)在乙肝肝硬化合并自发性细菌性腹膜炎(SBP)患者血清中的水平并探讨其临床价值。方法 70例乙肝肝硬化患者中,40例合并自发性细菌性腹膜炎患者设为SBP组,30例未合并自发性细菌性腹膜炎患者设为无SBP组。同期选择30例门诊健康体检者作为对照组。比较各组血清LPS、PCT水平;根据预后将SBP组患者分为好转组(n=21)和恶化组(n=19),比较两组血清LPS、PCT表达水平。结果 SBP组血清LPS、PCT表达水平均明显高于无SBP组及对照组(P<0.05),无SBP组血清LPS、PCT表达水平明显高于对照组(P<0.05);恶化组血清LPS、PCT表达水平均明显高于好转组(P<0.05)。结论血清LPS、PCT水平检测在乙肝肝硬化合并SBP患者的临床诊断及预后评估等方面具有重要的临床价值。

Objective To observe the levels of lipopolysaccharide （LPS） and procalcitonin （PCT） of the patients with HBV-liver cir- rhosis complicated with spontaneous bacterial peritonitis （SBP） and to discuss the clinical value. Methods Among 70 patients with HBV-liver cirrhosis, 40 patients were complicated with SBP （SBP group） and 30 patients were not complicated with SBP （non-SBP group）; 30 norm of physical examination were selected to be control group; the levels of LPS and PCT of the cases in the 3 groups were compared; patients in SBP group were subdivided, according to the prognosis, into 2 subgroups： improvement group （n=21） and deterio- ration group （n=19）, the levels of LPS and PCT of the patients in the 2 subgroups were compared. Results The levels of LPS and PCT of the patients in SBP group were much higher than those in non-SBP group and control group （P〈0.05）; the levels of the patients in non-SBP group were higher than those in control group （P〈0.05）; the levels of the patients in deterioration group were higher than those in improvement group （P〈0.05）. Conclusions The detection of the levels of LPS and PCT in patients with HBV-liver cirrhosis compli- cated with SBP is of important clinical value in diagnosis and prognosis evaluation.
