肺表面活性物质在新生儿重症肺炎治疗中的应用 被引量:2

摘要 目的:探究肺表面活性物质(PS)对于新生儿重症肺炎的临床治疗效果。方法:选取2015年5月-2016年5月笔者所在医院收治的60例重症肺炎新生儿,随机分成治疗组和对照组,每组30例,对照组给予常规通气治疗,治疗组在对照组基础上给予PS治疗,对两组患儿血氧相关指标、肺表面活性蛋白B(SP-B)、氧暴露时间和机械通气时间等进行比较;对两组患儿的临床疗效进行比较。结果:治疗组患儿在治疗24、48及72 h后氧合指数(OI)、动脉/肺泡氧分压比(a/PaO_2)、PaCO_2、血液pH和SP-B浓度均显著优于对照组,差异均有统计学意义(P<0.05);氧暴露时间和机械通气时间均显著短于对照组,差异均有统计学意义(P<0.05);治疗组治疗有效率达93.3%,显著高于对照组的66.7%,差异有统计学意义(P<0.05)。结论:肺表面活性物质用于新生儿重症肺炎的治疗是安全有效的,值得在临床上广泛应用。 Objective: To study the application of pulmonary surfaetant (pulmonary surfactant, PS) for clinical treatment of neonatal severe pneumonia. Method: 60 cases of neonatal pneumonia from May 2015 to May 2016 in the author's hospital were randomly divided into treatment group and control group, 30 cases in each group, the control group was treated with conventional ventilation treatment, the observation group was given PS treatment on the basis of control group, the Blood oxygen index, SP-B, oxygen exposure time and the duration of mechanical ventilation, and the clinical curative effect of two groups were compared.Result: The treatment group in the treatment of 24, 48 and 72 h after Oxygenation index(OI), Arterial/alveolar oxygen partial pressure ratio (a/PaO2), PaCO2, pH and SP-B concentrations in blood were significantly better than the control group , the difference was statistically significant(P〈0.05), oxygen exposure time and mechanical ventilation time were significantly shorter than the control group, the difference was statistically significant(P〈0.05).The effective rate of treatment group was 93.3%, significanfly higher than that of the control group with 66.7% , the difference was statistically signitlcant(P〈0.05).Condusiou: Pulmonary surfactant for the treatment of neonatal severe pneumonia is safe, it is worthy of clinical application.
