摘要
目的探讨2型糖尿病视网膜病变患者血清Fractalkine(FKN)水平与血脂、血糖的关系。方法根据眼底荧光造影,将126例2型糖尿病患者分为单纯糖尿病组背景期糖尿病视网膜病变组和增殖期糖尿病视网膜病变组,同时选择健康体检者40例为正常对照组(NC),测定各组血清FKN及空腹血糖(FPG)、餐后2 h血糖(2h PG)、糖化血红蛋白(Hb A1c)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、总胆固醇、三酰甘油,并分析血清FKN与相关指标的关系。结果 1血清FKN在2型糖尿病组高于NC组,且随着视网膜病变的出现及逐渐加重,各组间比较,差异有统计学意义(P<0.05)。2相关性分析显示,血清FKN与FPG、2 h PG、Hb A1c、LDL-C呈正相关,组间比较,差异有统计学意义。与HDL-C呈负相关,组间比较,差异有统计学意义。(P<0.05)。多元线性逐步回归分析显示,Hb A1c、LDL-C是FKN的独立危险因素。结论糖尿病视网膜病变患者的血清FKN随着病情加重而升高,与血糖、血脂代谢紊乱共同参与视网膜病变的发生、发展。
Objective To explore the relationships of serum fractalkine (FKN) with blood glucose and lipid in the patients with type 2 diabetic retinopathy (DR). Methods A total of 126 patients with type 2 diabetes were divided into three groups, i.e. no retinopathy group, background DR group and proliferative DR group based on the fundus fluorescein angiography; 40 healthy volunteers were recruited as normal control group. The serum levels of FKN, fasting plasma glucose (FPG), 2-h postprandial plasma glucose (2hPG), glycosylated hemoglobin (HbA1), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), total cholesterol (TC) and triglycerides (TG) were measured. The relationships of FKN level with the biochemical indices were analyzed using Preason correlation. Results The levels of serum FKN in the type 2 diabetic groups were significantly higher than that in the normal control group, and increased with the occurrence and exacerbation of retinopathy; the differences were statistically significant (P〈 0.05). Serum FKN was positively correlated with FPG, 2hPG, HbA1c and LDL -C (r= 0.293, 0.382, 0.355 and 0.402; P〈 0.01 or P〈 0.05) and negatively related to HDL-C (r=-0.307, P〈0.05). HbAlc and LDL-C were the independent risk factors affecting serum concentration of FKN. Conclusions FKN level significantly increases with development of DR.Together with the disorder of lipid metabolism and blood glucose, it participates in the occurrence and development of diabetic retinopathy.
