人性化护理干预结合湿性愈合疗法在乳腺癌压疮患者中的应用效果

Effects of Humanized nursing intervention combined with moist wound healing in treatment of pressure sore of breast cancer patients

目的分析和研究人性化护理干预结合湿性愈合疗法在乳腺癌压疮患者中的应用效果。方法连续选择2013年3月—2014年7月哈尔滨医科大学附属肿瘤医院收治的乳腺癌住院患者50例(Ⅱ期、Ⅲ期压疮),男性20例,女性30例,平均年龄(63.0±2.5)岁;随机将其分为对照组与试验组各25例。对照组采用传统的压疮治疗方法,试验组采用人性化护理干预结合湿性愈合疗法。治疗28 d后对比分析2组乳腺癌住院压疮患者的治疗效果及其换药情况。结果试验组患者的好转率显著高于对照组,差异有统计学意义(92.0%vs.60.0%,χ~2=5.031,P<0.001),其中试验组中Ⅱ期与Ⅲ期压疮患者好转率均显著高于对照组,亚组比较差异均有统计学意义(92.31%vs.71.43%,χ~2=4.237,P<0.001;91.67%vs.45.45%,χ~2=4.932,P<0.001)。治疗后试验组患者的压疮Branden评分和压疮面积与对照组比较均显著减少,差异均有统计学意义(P<0.05)。试验组患者的换药次数、换药时间及住院的平均费用与对照组比较均显著降低,差异有统计学意义(P<0.001)。结论人性化护理干预结合湿性愈合疗法在防治乳腺癌住院患者压疮效果相对较好,值得临床推广。

Objective To study the effects of humanized nursing intervention combined with moist wound healing in treat- ment of pressure sore of breast cancer patients. Methods A total of 50 breast cancer inpatient with pressure sore in our hospital from March,2013 to July,2014,20 males and 30 female,with a mean age of （63.0 _＋ 2.5 ）years, were randomly divided into control group and experiment groups with 25 cases in each group. The patients in the control group received traditional pressure sore treatments, while the patients in the experiment group received Humanized nursing intervention combined with moist wound healing therapy. The clinical efficacy was compared between the two groups after the treatment of 28 days. Results The effective rate in the experiment group was significantly higher than that in the control group （92.0% vs. 60.0% ,χ2 =5. 031 ,P 〈0. 001） ;the effective rate of stage I1 and 111 patients in the experiment group was significantly higher than that in the control group, and the difference between the subgroups was significant（92.31% vs. 71.43 % , χ2 = 4. 237, P 〈 0. 001 ;91.67 % vs. 45.45 % , χ2 = 4. 932, P 〈 0.001 ）. Branden score and area of pressure sore after the treatment in the experiment group were both significantly lower than those in the control group （ P 〈 0.05 ）. The frequency and time of dressing change and average hospital cost in the experiment group were all significantly less than those in the control group （P 〈 0. 001 ）. Conclusion Humanized nursing intervention combined with moist wound healing therapy may greatly improve the pressure sore in hospitalized breast cancer patients.
