牙隐裂保存治疗临床探讨

目的探讨需要做根管治疗的牙隐裂牙齿保存治疗获得成功的方法。方法临床选择75颗需要做根管治疗的牙隐裂牙齿,经调牙合、根管治疗、复合树脂充填、临时冠修复、全冠修复等治疗,随访。结果 57颗牙保存成功(时间≥2年),18颗牙治疗失败。结论通过观察裂纹范围、判断牙周状况及其与裂纹的关系、判断能否做全冠修复等方法严格选择适应证,通过使用橡皮障、调磨对颌牙、术中戴临时冠、根管治疗后全冠修复等方法预防劈裂,是需要做根管治疗的牙隐裂牙齿保存治疗获得成功的关键方法。

Objective To investigate the clinical methods of preserving cracked teeth that need to have a root canal therapy. Methods 75 cracked teeth that needed to have a root canal therapy were included. These cases were treated with occlusal adjustment, root canal therapy, resin restoration, temporary crown restoration, and full crown restoration. And then follow-up results were evaluated. Results The results were satisfactory in 57 teeth （≥2 years） and failure in 18 teeth after treatment. Conclusion Indications should be restricted according to crack range, periodontal condition, the relevance between the former two, and full crown restoration. The use of rubber dam, occlusal adjustment, temporary crown restoration and complete coronal restoration after root canal therapy can be effective in preventing teeth fracture. Strict indications and teeth fracture prevention are the key of a successful therapy.
