自拟健骨方治疗绝经后骨质疏松性椎体压缩骨折临床疗效观察及对患者OPG/RANKL/RANK的影响

Clinical Curative Observation of Using Self-made Jiangu Recipe in the Treatment of Postmenopausal Osteoporotic Vertebral Compression Fracture and its Influence on OPG/RANKL/RANK of the Patients

目的:观察自拟健骨方治疗绝经后骨质疏松性椎体压缩性骨折的临床疗效及其作用机制的研究。方法:选取81例绝经后骨质疏松性椎体压缩性骨折患者随机分为2组(对照组40例,观察组41例)。2组均采用基础保守治疗,对照组患者采用复方骨肽注射液,肌内注射,4ml/次,1次/日,连续治疗6个月;观察组采用自拟健骨方治疗,每日1剂,分早晚2次温服,连续治疗6个月。比较2组患者治疗前后血清中骨碱性磷酸酶(B-ALP)和骨保护素(OPG)及核因子κB受体活化因子配基(RANKL)的含量,同时比较2组患者的临床疗效。结果:给药后2组患者血清中的B-ALP和RANKL的含量,及RANKL/OPG的水平在治疗后均显著降低(P<0.05),而OPG则显著升高(P<0.05);并且观察组血清中B-ALP和RANKL的含量,及RANKL/OPG的水平均明显低于对照组(P<0.05),OPG则明显高于对照组(P<0.05)。治疗后对照组的总有效率为82.5%,观察组的总有效率为97.6%,2组的临床疗效具有显著性差异(P<0.05)。结论:自拟健骨方对绝经后骨质疏松性椎体压缩骨折治疗的疗效确切,值得在临床上推广。

Objective： To observe the clinical curative effect of using Self-made Jiangu Recipe in the treatment of postmenopausal osteoporosis vertebral compression fracture and its mechanism of action. Methods： 81 cases of postmenopausal osteoporosis vertebral compression fracture patients were randomly assigned to two groups,40 cases in the controlled group,the observationgroup had 41 cases,two groups were both used based conservative treatment,the controlled group was treated with compound ossotide injection,intramuscular injection,4ml / times,1time / D,continuous treatment for 6months; the observation group used Self-made Jiangu Decoction,daily 1agent,sooner or later 2lukewarm took orally,continuous treatment for 6months. Compared between the two groups before and after treatment in patients with serum bone specific alkaline phosphatase（ b-alp） and bone protection element（ OPG） and nuclear factor kappa B receptor activation factor ligand（ RANKL） content and also were compared between the two groups of the clinical curative effect. Results： After the administration of the content of 2groups of patients with serum b-alp and RANKL and RANKL / OPG levels after treatment were significantly lower（ P〈0. 05）,but the OPG significantly increased（ P〈0. 05）; and to observe the content of serum b-alp and RANKL and RANKL / OPG levels were significantly lower than the controlled group（ P〈0. 05） OPG was significantly higher than that of the controlled group（ P〈0. 05）. The total effective rate of the controlled group was 82. 5%,the total effective rate of the observation group was 97. 6%,and the clinical effect of the2 groups was significantly different（ P〈0. 05）. Conclusion： The curative effect of Self-made Jiangu Recipe for postmenopausal osteoporotic vertebral compression fracture is effective,worthy of clinical popularization.
