大黄牡丹汤联合腹腔镜治疗急性阑尾炎临床疗效观察

Clinical Curative Observation of Using Dahuang Mudan Decoction Combined with Laparoscope in the Treatment of Acute Appendicitis

目的:观察大黄牡丹汤联合腹腔镜治疗急性阑尾炎的临床疗效;方法:将行腹腔镜阑尾切除术的121例患者,随机分为2组。对照组61例予以单纯腹腔镜治疗;治疗组60例在对照组治疗基础上加服大黄牡丹汤。比较两组治疗后的疗效,中医证候积分、功能恢复及并发症发生情况,监测不良反应。结果:治疗组总有效率为93.33%,与对照组(80.33%)相比,具有明显差异(P<0.05);治疗后两组中医证候积分均明显下降,与治疗前相比,差异显著(P<0.05),治疗组较对照组下降更明显;治疗组肠鸣音恢复时间、总住院时间均短于对照组,并发症较对照组发生率低,差异均具有统计学意义(P<0.05);两组患者在治疗过程中均未发现不良反应。结论:大黄牡丹汤联合腹腔镜治疗急性阑尾炎疗效确切,可明显改善症状,减少并发症,缩短住院时间,安全可靠。

Objective： To observe therapeutic effect of using Dahuang Mudan Decoction combined with laparoscopic in the treatment of acute appendicitis. Methods： The laparoscopic resection of 121 patients,were randomly assigned to two groups：61cases in controlled group,treated with laparoscopic treatment only; 60 cases in the treatment group were treated with Dahuang Mudan Decoction on the basis of the controlled group. Comparing the effects of the two groups after treatment,TCM syndrome score,functional recovery and complications and adverse reaction monitoring. Results： The total efficiency of the treatment group was 93. 33%,compared with 80. 33% in the controlled group,with significant difference（ P〈0. 05）; the two groups after treatment of TCM syndrome integral were significantly decreased,compared with before treatment,the difference was significant（ P〈0. 05）,the treatment group decreased more significantly than the controlled group; the treatment group,the recovery time of bowel sound,total hospitalization time was shorter than the controlled group,the complication rate was low,the differences were statistically significant（ P〈0. 05）; the two groups of patients during treatment were not adverse reaction was found. Conclusion：It has curative effect of using Dahuang Mudan Decoction combined with laparoscopy in treating acute appendicitis,can significantly improve symptoms,reduce complications,shorten the hospitalization time,safe and reliable.
