摘要
目的分析2型糖尿病合并急性心肌梗死患者的相关危险因素。方法对2型糖尿病合并急性心肌梗死患者50例(A组)、单纯急性心肌梗死50例(B组)、单纯2型糖尿病患者50例(C组)间空腹血糖(FBG)、血压、血脂、血凝指标纤维蛋白原(FIB)等危险因素进行分析,并根据冠状动脉病变支数不同将A组分为1支病变(A1组)12例、2支病变(A2组)16例、3支病变(A3组)22例,比较各亚组之间FBG、血脂、FIB浓度变化情况。结果 A组和B组的体质量指数(BMI)与C组相比较,差异有统计学意义(P<0.05)。A组、B组中的空腹血糖(FBG)、胆固醇(CHO)、甘油三酯(TG)、低密度脂蛋白(LDL)、纤维蛋白原(FIB)显著高于C组,高密度脂蛋白(HDL)显著低于C组;A组与B组间TG、FBG值差异有统计学意义,且A组明显高于B组。A组中FIB在A1亚组、A2亚组与A3亚组间比较差异有统计学意义;A3组的FIB值较A1组、A2组明显升高,血脂及血糖变化在3个亚组间差异无统计学意义(P>0.05)。结论血糖、血压、血脂异常及高凝状态可能使2型糖尿病患者急性心肌梗死的发生率升高,故尽早在上述方面做好预防和控制,可延缓和减少糖尿病患者心血管事件的发生。
Objective To analysis the risk factors of type 2 diabetes complicatingacute myocardialinfarction. Methods The patients was divided into three gruoups: 50 cases of type 2 diabetes complicatingacutmyocardialinfarction group( group A),50 cases of simple acute myocardial infarction group( group B),50 cases of simple type 2 diabetes mellitus group( group C),to detectblood glucose,blood pressure,blood lipid,hypercoagulability of these three groups. Group A was divided into 1 lesion group( including 12 cases),2 lesionon subgroup( including 16 cases) and 3 lesion group( including 22 cases) according to the difference pathological changes,and comparing the change of the concentration of fast blood glucose,blood lipid and plasma FIB among subgroups. Results BMI、FBG、CHO、TG、LDL、FIB in group A and group B were significantly higher than group C( P〈0. 05),HDL in group A and group B was significantly lower than the group C( P〈0. 05),the values of TG and FBG in group A was significantly different from group B( P〈0. 05),the blood pressure of group A and group C were significantly differnernt( P〈0. 05),but the ageof group A,group B and group C have no significancely different( P〉0. 05). FIB values of 3 lesionin group A was higher than 1 lesion and 2 lesions( P〈0. 05). In subgroups there was nosignificantly different in fast blood glucose and blood lipid( P〉0. 05). Conclusions Diabetes,hypertension,dyslipidemia,hypercoagulability can increase higher incidence of type 2 diabetes complicatingacutmyocardialinfarction,so as early as possible taking measures to preventing and reducing the cardiovascular disease.
