非酒精性脂肪肝病合并胰岛素抵抗患者血清chemerin和hs-CRP水平的研究

Study of serum chemerin and hs-CRP in patients with nonalcoholic fatty liver disease complicating insulin resistance

目的测定非酒精性脂肪肝病(NAFLD)患者血清中视黄酸受体反应蛋白2(chemerin)、超敏C反应蛋白(hs-CRP)的表达水平,以及胰岛素抵抗指数(HOMA-IR),探讨chemerin在NAFLD发病中的机制。方法将NAFLD患者分为两组,NAFLD组(30例)和NAFLD+IR组(30例);并选择健康体检者30例作为健康对照组。用双抗体夹心酶联免疫吸附法检测血清chemerin,免疫比浊法检测血清hs-CRP,同时测定空腹血糖(FPG)、空腹胰岛素(FINS),并计算胰岛素抵抗指数(HOMA-IR)。结果与健康对照组相比较,NAFLD组患者的血清chemerin和hs-CRP水平明显升高(P<0.05)。与健康对照组和NAFLD组比较,NAFLD+IR组患者的血清chemerin和hs-CRP水平也明显升高(P<0.05)。血清chemerin水平与hs-CRP、HOMA-IR呈显著正相关。结论 NAFLD患者的血清chemerin水平与hs-CRP、HOMA-IR相关。

Objective To detect the serum levels of chemerin and high-sensitivity C-reactive protein（hs-CRP）in the patients with nonalcoholic fatty liver disease（NAFLD）and homeostasis model assessment of insulin resistance（HOMA-IR）,and to investigate the mechanism of chemerin in NAFLD.Methods The patients with NAFLD were divided into the NAFLD group（30cases）and the NAFLD＋IR group（30cases）;other 30 individuals undergoing the healthy physical examination were selected as the healthy control group.The serum levels of chemerin and hs-CRP were measured by ELISA and immunoturbidimetry,respectively.Meanwhile,the fasting plasma glucose（FPG）and fasting insulin（FINS）were detected,and HOMA-IR was calculated.Results The levels of serum chemerin and hs-CRP in the NAFLD group were increased obviously compared with the healthy control group（P〈0.05）.The levels of serum chemerin and hs-CRP in the NAFLD＋IR group were significantly increased compared with the NAFLD group and healthy control group（P〈0.05）.The serum chemerin level was positively correlated with the hs-CRP level and HOMAIR.Conclusion The serum chemerin level is correlated with hs-CRP and HOMA-IR in NAFLD patients.
