清营化痰法治疗老年糖尿病患者肺部感染临床观察

Clinical Observation on the Treatment of Pulmonary Infection in Elderly Patients with Diabetes by Clearing Ying and Dispelling Phlegm Method

目的:观察清营化痰法治疗老年糖尿病患者肺部感染的临床疗效。方法:按照1∶1随机原则,将83例60岁以上糖尿病并发肺部感染者分为治疗组42例和对照组41例。在对照组西医常规治疗基础上,治疗组加清营化痰法治疗,处方以清营汤合涤痰汤加减。疗程均为14 d。结果:治疗组总有效率90.48%,对照组73.17%,有统计学意义。两组治疗后中医证候积分均较治疗前明显下降,有显著统计学意义,但治疗组更优于对照组。两组治疗后白细胞总数、中性粒细胞比率、氧分压比较,差异均有统计学意义。两组治疗后二氧化碳分压比较,差异无统计学意义。结论:治疗组治疗效果优于对照组。两组治疗均明显改善了中医症状,但治疗组更优于对照组。治疗组在白细胞总数、中性粒细胞比率、氧分压指标改善方面优于对照组,在二氧化碳分压改善方面未表现出差异。

Objective：To observe the clinical curative effect on the treatment of pulmonary infection in elderly patients with diabetes by clearing Ying and dispelling phlegm method. Methods：According to the random principle of 1∶1,83 patients of diabetes complicated with pulmonary infection over 60 years old were divided into 42 cases in the treatment group and 41 cases in the control group. On the basis of routine treatment of Western medicine in the control group,the treatment group added the therapy of clearing Ying and dispelling phlegm,prescription combined Qingying Decoction and Ditan Decoction. The course of treatment was 8 weeks. Results：The total effective rate of the treatment group was 90.48%,and the control group was 73.17%,which was statistically. After treatment,the scores of syndrome in the two groups were significantly decreased and statistically significant compared with before treatment,but the treatment group was better than the control group. White blood cell count,neutrophil ratio and partial pressure of oxygen in the two groups were compared after treatment,and the difference was statistically significant. After treatment,there was no significant difference in partial pressure of carbon dioxide between the two groups. Conclusions：The clinical curative effect of the treatment group was better than the control group. Two groups were significantly improved the symptoms of Chinese medicine,but the treatment group was better than the control group. The treatment group was better than the control group in the improvement of white blood cell count,neutrophil ratio and partial pressure of oxygen,and showed no difference in partial pressure of carbon dioxide.
