摘要
目的检测多囊卵巢综合征(PCOS)患者卵巢颗粒细胞中microRNA-200b(miR-200b)的表达,分析其与胰岛素抵抗的关系。方法选取PCOS患者120例(其中胰岛素抵抗78例,非胰岛素抵抗42例)及对照患者72例。采用逆转录定量PCR(qRT-PCR)检测在卵巢颗粒细胞中miR-200b的表达,并分析其临床意义。结果 PCOS胰岛素抵抗患者中超重或肥胖、高血压、高血糖、脂代谢紊乱和代谢综合征的发生率皆高于PCOS非胰岛素抵抗患者和对照患者(P均<0.05)。PCOS胰岛素抵抗患者miR-200b表达上调,与PCOS非胰岛素抵抗患者和对照患者比较差异有统计学意义(P=0.012,P<0.001);且PCOS胰岛素抵抗患者中miR-200b表达量与胰岛素抵抗指数(HOMA-IR)呈正相关(r=0.532,P=0.002)。结论 PCOS胰岛素抵抗患者高血压等代谢方面远期并发症的患病率较高。miR-200b高表达与PCOS胰岛素抵抗呈正相关,可能是PCOS患者出现胰岛素抵抗的重要病理机制之一。
Objective To explore the expression and clinicopathological significance of microRNA-200b( miR-200b)in ovarian granulosa cells of patients suffering from polycystic ovary syndrome( PCOS) with insulin resistance.Methods A total of 120 PCOS patients( 78 with insulin resistance and 42 without) and 72 controls were enrolled. The expression of miR-200 b was examined in ovarian granulosa cells by real-time quantitative reverse transcription PCR( qRT-PCR),and the clinical significance was analyzed. Results PCOS patients with insulin resistance had higher morbidity of overweight,obesity,hypertensive disease,hyperglycemia,dyslipidemia and metabolic syndrome than PCOS patients without insulin resistance and controls( all P〈0. 05). Expression of miR-200 b was much higher in PCOS patients with insulin resistance than in PCOS patients without insulin resistance and controls( P = 0. 012,P〈0. 001). In PCOSpatients with insulin resistance,the index of homeostatic model assessment of insulin resistance( HOMA-IR) was positively correlated with miR-200 b expression( r = 0. 532,P = 0. 002). Conclusion PCOS patients with insulin resistance have high morbidity of long-term metabolic complications. The high expression of miR-200 b is positively related to insulin resistance,which may be the underlying pathogenic mechanism for insulin resistance in PCOS patients.
