肺康宁合剂对慢性阻塞性肺疾病肺动脉高压患者内皮素和D-二聚体的影响

Feikangning Mixture on Endothelin and D-dimmer of Chronic Obstructive Pulmonary Disease Patients with Pulmonary Hypertension

目的:探讨肺康宁合剂治疗慢性阻塞性肺疾病肺动脉高压患者临床疗效,并观察其对内皮素-1(ET-1)、D-二聚体的影响。方法:回顾性分析医院52例慢性阻塞性肺疾病肺动脉高压患者临床诊治情况,其中以常规治疗者为对照组(21例);于常规治疗基础上采用肺康宁合剂治疗者为治疗组(31例)。检测两组患者治疗前、后ET-1、D-二聚体及肺动脉压变化,统计两组临床疗效及临床症状、体征(咳嗽、喘息、咳痰及哮鸣音)积分变化,观察治疗期间不良反应。结果:治疗后,两组患者ET-1、D-二聚体及肺动脉压水平均较治疗前显著下降,治疗组各项指标下降程度明显优于对照组,P<0.05;两组患者治疗后咳嗽、喘息、咳痰及哮鸣音积分较治疗前明显下降,治疗组各项症状、体征积分下降程度优于对照组,P<0.05。治疗组治疗总有效率90.32%,明显高于对照组57.14%,P<0.05;治疗期间未见严重性不良反应。结论:采用肺康宁合剂治疗慢性阻塞性肺疾病肺动脉高压可显著改善其肺动脉高压及临床各项症状、体征,提高临床疗效且具有一定安全性,然该药物作用机制可能与其改善患者血管内皮功能及高凝状态有一定关系。

Objective ： To investigate the clinical efficacy of Feikangning Mixture on endothelin - 1 （ ET - 1 ） and D - dimmer of chronic obstructive pulmonary disease patients with pulmonary hypertension. Methods ： Retrospectively analyze clinical diagnosis and treatment of 52 cases of chronic obstructive pulmonary disease patients with pulmonary hypertension in our hospital. The conventional treatment control group had 21 cases. Based on the conventional treatment, the treatment group（51 cases） used Feikangning Mixture. Before and after treatment, detect ET- 1, D- dimmer and pulmonary artery pressure changes. Statistically analyze clinical efficacy and clinical symptoms and signs （ cough, wheezing, sputum and wheezing） integral changes during treatment and observe the adverse reactions. Results： After treatment, the two groups＇ ET - 1, D - dimmer and pulmonary arterial pressure levels decreased significantly than those before treatment and the treatment group＇ s decrease was more obvious than the control group ＇ s, P 〈 0. 05. Two groups＇ cough, wheezing, sputum and wheezing sound significantly decreased after treatment. The treatment group＇ s decline degree was more obvious than that of the control group, P 〈0. 05. The treatment group＇s total effective rate was 90.32%, significantly higher than 57.14% in the control group, P 〈 0. 05. There was no severe adverse reactions during treatment. Conclusion：When treating chronic obstructive pulmonary disease and pulmonary hypertension, Feikangning Mixture can significantly improve pulmonary hypertension and clinical symptoms, signs and the clinical efficacy and has a certain security. The mechanism may be related to improvement of vascular endothelial function and hypercoagulable state.
