Analysis of Causes of Crossmatching Incompatibility in Patients With Hematological Diseases

目的 探讨血液病患者疑难交叉配血的原因及输血对策。方法对疑难配血标本进行血型鉴定、不规则抗体筛查和鉴定等并对数据进行统计分析。结果 血液病患者凝聚胺交叉配血不合的原因中,不规则抗体阳性最多见,占82.4%（14/17）,多以同种抗体产生为主,占85.7%（12/14）,其中Rh系统抗体占66.7%（8/12）;输血5次以上的血液病患者不规则抗体检出率高于输血1-5次的患者。结论 对血液病患者,输血前筛查不规则抗体十分重要,可有效提高输血质量与安全。

Objective To investigate the causes of crossmatching incompatibility in patients with hematological diseases. Methods Blood type identification, irregular antibody screening and identification were detected in blood specimens of crossmatching incompatibility. Results The most common causes of crossmatching incompatibility in patients with hematological diseases were the positive of irregular antibody, accounted for 82.4%（14/17）, in which 12 cases were identified as specific alloantibody, accounted for 85.7%（12/14）. Among 12 cases consisting of specific alloantibodies, 8 indentified antibodies had Rh blood group system, accounting for 66.7%（8/12）. Besides, 5 times the number of blood transfusion, the higher 1-5 times the rate of irregular antibodies. Conclusion Irregular antibodies screening before blood transfusion is very important, especially for the patients with hematological diseases. Irregular antibodies screening can effectively improve the quality and safety of blood transfusion
