临时起搏器植入术后的护理研究

Nursing Research of Temporary Pacemaker Implantation

目的 评价缓慢型心律失常临时起搏器术后的护理结局,总结护理经验。方法 2012年10月-2016年7月,医院共采用临时起搏器植入治疗缓慢型心律失常38例。结果 3例患者死亡,死亡率7.89%,2例患者自动出院,其余患者均正常出院;植入后、出院前各时间段SBP、DBP、HR均高于术前,差异有统计学意义（P〈0.05）;未见导管断裂、感染,未出现导管移位、膈肌刺激,3例死亡患者死于心源性休克。结论 缓慢型心律失常植入临时起搏器可快速回复正常的心率,纠正心律失常,从而降低死亡风险,但有部分患者可能失败,需加强术后监护、管理。

Objective To evaluate the nursing outcomes after the operation of the temporary pacemaker with slow arrhythmia, and summarize nursing experience. Methods A total of 38 patients with slow arrhythmia were treated by temporary pacemaker implantation from October 2012 to July 2016. Results 3 patients died, the mortality rate was 7.89%, 2 patients were discharged from hospital, the other patients were normal discharge. After implantation, prior to discharge each time SBP, DBP and HR were higher than before, the difference was statistically significant（P〈0.05）; No infection, catheter fracture, without catheter displacement and diaphragm stimulation, 3 patients died of cardiogenic shock. Conclusion The temporary pacemaker can quickly restore normal heart rate and correct arrhythmia, so as to reduce the risk of death, but some patients may fail, need to strengthen postoperative monitoring and management.
