预防误吸集束化方案在人工气道患者肠内营养中的应用

Application of bundled aspiration prevention strategy during enteral nutrition for patients with artificial airway

目的探讨预防误吸集束化方案在人工气道患者肠内营养中的应用效果。方法 386例ICU人工气道患者随机分成实验组和对照组,各193例。2组均给予肠内营养,对照组采用常规护理,实验组在对照组基础上采用预防误吸集束化方案。比较2组呼吸道分泌物p H值,观察2组吸入性肺炎及误吸发生率。结果实验组呼吸道分泌物p H值高于对照组,差异有统计学意义(P<0.05),实验组误吸和吸入性肺炎发生率分别为3.11%和4.14%,低于对照组的11.40%和12.43%,差异有统计学意义(P<0.01)。结论集束化方案有效预防人工气道患者肠内营养治疗期间的误吸,从而降低吸入性肺炎的发生率。

Objective To evaluate the efficacy of bundled aspiration prevention strategy dur-ing enteral nutrition in ICU patients with artificial airway. Methods A total of 386 ICU patients with artificial airway were randomly assigned into control group and study group, with 193 cases in each group. A bundled strategy was applied during enteral nutrition in the study group while the control group received regular enteral nutrition. The pH level of airway secretions, incidences of aspiration and aspiration pneumonia were recorded and compared between two groups. Results The study group had a higher mean pH compared with the control group （P 〈 0. 05 ） . The incidence of aspiration and aspiration pneumonia was 3.11% and 4. 14% in the study group respectively, and was 11.40% and 12. 43% in the control group respectively, with significant difference （P 〈0. 01 ） . Conclusion The bundled strategy can effectively prevent aspiration during enteral nutrition and reduce aspiration pneu-monia in patients with artificial airway.
