快速康复外科理念在全膝关节置换术后早期康复中的应用

Application of fact-track surgery in early rehabilitation after total knee arthroplasty

目的探讨快速康复外科理念在全膝关节置换(TKA)术后早期康复中的应用效果。方法 40例行TKA治疗的患者随机分为实验组和对照组,各20例。对照组常规护理,实验组在对照组基础上运用快速康复外科理念指导康复。观察术后1周、2周及1个月膝关节功能恢复情况,记录2组住院时间及持续被动功能(CPM)训练时间。结果实验组术后1周、2周及1个月HSS评分均高于对照组,差异有统计学意义(P<0.01),且实验组住院时间和CPM训练时间均短于对照组,差异有统计学意义(P<0.05或P<0.01)。结论快速康复外科理念有助于提升TKA术后康复质量,降低住院时间。

Objective To investigate the effect of fact-track surgery in early rehabilitation af-ter total knee arthroplasty （ TKA） . Methods A total of 40 patients with TKA were randomly divided into the study group and the control group, with 20 cases in each group. Both group received routine nursing care during treatment, and the fact-track surgery theory-based interventions were applied in the study group. The knee function was evaluated by hospital for special surgery （ HSS） at 1 week, 2 weeks and 1 month after surgery. The length of stay and time of continuous passive motion training （CPM） were recorded. Results The HSS score was higher in the study group than that of controls at 1 week, 2 weeks and 1 month after surgery respectively, with significant difference （P 〈0.01）. Pa-tients in the study group had shorter length of stay and shorter duration of CPM training compared with those in the control group, with significant difference （P 〈0.05 o r P 〈0.01）. Conclusion Nursing interventions based on fact-track surgery can improve the postoperative rehabilitation and shorten the length of stay in patients after TKA.
