“四乌汤”加减治疗幽门螺杆菌(Hp)相关消化性溃疡的临床研究

Clinical study on the treatment of Hp related peptic ulcer with "Four Wu Tang"

目的:观察"四乌汤"加减治疗幽门螺杆菌(Hp)相关消化性溃疡的临床疗效。方法:选取60例符合诊断标准的患者,随机分为治疗组及对照组,各30例,治疗组以自拟"四乌汤"为基本方随证加减;对照组用泮托拉唑钠肠溶片40 mg,2次/d,阿莫西林分散片1 000 mg,2次/d,甲硝唑片0.4 g,2次/d,口服根除Hp治疗10 d,后继续给予泮托拉唑钠肠溶片40 mg,1次/d,口服抗溃疡治疗,总疗程8周。两组资料治疗前、后均行Hp定性检测、胃镜检查,其中对照组在常规根治Hp治疗结束后复查Hp定性,如仍为阳性,抗HP疗程增至14 d,后继续抗溃疡治疗,总疗程不变。结果:治疗组临床总有效率为96.6%,显著优于对照组的80.0%,差异有统计学意义(P<0.05)。结论:自拟"四乌汤"加减治疗Hp相关消化性溃疡具有明确临床效果,溃疡愈合速度、程度、临床症状缓解度均较对照组有一定优势,值得临床推广。

Objective To observe the clinical effect of ＂ Four Wu Tang ＂ in the treatment of Hp related peptic ul-cer. Method 60 cases who match the diagnosis standard were divided into two groups with 30 cases in each group at random according to the statistical method. The treatment group with self-made ＂ Four Wu Tang ＂ decoction and its modified formulas;the control group were given oral pantoprazole sodium enteric coated tablets 40 mg bid,amoxicillin dispersible tablets 1 000 mg bid and metronidazole tablets 0. 4 g bid for Hp eradication treatment for 10 days,and continued to give oral pantoprazole sodium enteric coated tablets 40 mg QD for anti ulcer treatment. Total course was 8 weeks. Before and after treatment,data of two groups were performed with Hp qualitative detection,gastroscopy. After the conventional radical treatment of Hp,if the reexamination of Hp qualitative of the control group,still was positive,then the anti Hp treatment increased to 14 days. And anti ulcer therapy was continued. The total course was unchanged. Results The total effective rate of the treatment group was 96. 6%,which was significantly better than that of the control group 80. 0%,the difference was statistically significant（ P〈0. 05）. Conclusion The self-made ＂ Four Wu Tang＂ decoction for the treatment of Hp related digestive ulcer has clear clinical effect.Healing rate,healing degree,clinical symptoms ease degrees of the ulcer lower than those in the control group has certain advantages. It is worth clinical promotion.
