视网膜母细胞瘤患者细胞免疫功能状态的改变

The changes of cellular immunity function in patients with retinoblastoma

目的研究视网膜母细胞瘤患者外周血T淋巴细胞亚群CD3^+、CD3^+CD4^+、CD3^+CD8^+细胞比例及CD3^+CD4^+/CD3^+CD8^+比值的变化,并探讨其临床意义。方法回顾性分析2015年4月至2016年4月在该院就诊的122例视网膜母细胞瘤患者外周血T细胞亚群变化,将视网膜母细胞瘤患者分为3组:双眼视网膜母细胞瘤组(n=51)、左眼视网膜母细胞瘤组(n=33)、右眼视网膜母细胞瘤组(n=38),另选取同期健康体检者45例作为健康对照组,使用Backman Coulter FC-500流式细胞仪,采用多参数流式细胞术检测各组的外周血T细胞亚群水平,对检测结果进行统计学分析。结果与健康对照组相比,视网膜母细胞瘤患者外周血CD3^+、CD3^+CD4^+细胞比例明显减少,差异有统计学意义(P<0.01);CD3^+CD8^+细胞比例明显升高,差异有统计学意义(P<0.01);CD3^+CD4^+/CD3^+CD8^+比值明显下降,差异有统计学意义(P<0.01);视网膜母细胞瘤患者各组间外周血T细胞亚群水平差异无统计学意义(P>0.05)。结论视网膜母细胞瘤患者有细胞免疫功能紊乱,监测其外周血T淋巴细胞亚群,可以评估患者的细胞免疫功能及疾病的状态,为临床治疗提供依据。

Objective To investigate the changes of CD3~＋,CD3~＋CD4~＋,CD3~＋CD8~＋ratio and CD3~＋CD4~＋/CD3~＋CD8~＋ratio of peripheral blood T lymphocyte subsets in patients with retinoblastoma,and to evaluate its clinical significance.Methods The clinical data of 122 retinoblastoma cases and 45healthy（healthy control group）were selected from April 2015 to April 2016 in Beijing Tongren Hospital were retrospective reviewed.The retinoblastoma patients were divided into 3groups：bilateral retinoblastoma group（51cases）,left retinoblastoma group（33cases）,right retinoblastoma group（38cases）.Backman Coulter football club-500 flow cytometry was used,multi-parameter flow cytometry was used to detect the level of T lymphocyte subsets in the peripheral blood of each group,and the results were statistically analyzed.Results The percentage of CD3~＋and CD3~＋CD4~＋cells in peripheral blood of patients with retinoblastoma was significantly lower than that of healthy control group（P〈0.01）,CD3~＋CD8~＋cells were significantly increased（P〈0.01）,CD3~＋CD4~＋/CD3~＋CD8~＋ratio was significantly decreased,the difference was statistically significant（P〈0.01）.There was no significant difference in the levels of T cell subsets in the peripheral blood of the patient′s with retinoblastoma（P〉0.05）.Conclusion Patients with retinoblastoma have cellular immune dysfunction,monitoring of peripheral blood T lymphocyte subsets,which can assess the patient′s cellular immune function and disease status,provide the basis for clinical treatment.
