复方血栓通胶囊联合卡波金对糖尿病视网膜病变患者血清炎症因子及视野缺损的影响

Effect of compound Xueshuantong capsule combined with carbogen on serum inflammatory factors and visual field defects in patients with diabetic retinopathy

目的观察复方血栓通胶囊联合卡波金对糖尿病视网膜病变患者血清炎症因子及视野缺损的影响。方法将72例糖尿病视网膜病变患者随机分为2组。对照组36例予羟苯磺酸钙片治疗,治疗组38例在对照组治疗基础上加用复方血栓通胶囊联合卡波金治疗。2组均治疗5个月后统计临床疗效,并观察2组治疗前后视野缺损、空腹血糖(FPG)、餐后2 h血糖(2h PG)、糖化血红蛋白(HbA1c)、血浆同型半胱氨酸(Hcy)、血清胱抑素C(Cys C)、超敏C反应蛋白(hs-CRP)、血清转化生长因子β1(TGF-β1)、白细胞介素-6(IL-6)及血清谷胱甘肽过氧化物酶(GSH-Px)变化情况。结果治疗组总有效率明显高于对照组(P<0.05)。2组治疗后视野灰度值、血管瘤体积均显著降低(P均<0.05);对照组治疗后出血斑面积、黄斑水肿无明显变化(P均>0.05),而治疗组均显著降低(P均<0.05);治疗组视野灰度值、血管瘤体积、出血斑面积及黄斑水肿降低程度均优于对照组(P均<0.05)。2组治疗后FPG、2h PG、HbA1c、Cys C、Hcy、TGF-β1及炎性因子hs-CRP、IL-6含量均较治疗前显著降低(P均<0.05),血清GSH-Px水平均显著升高(P均<0.05),且治疗组上述指标改善情况均明显优于对照组(P均<0.05)。结论复方血栓通胶囊联合卡波金治疗糖尿病视网膜病变疗效显著,可能与抑制炎性因子表达、降低Hcy及TGF-β1水平、升高GSH-Px水平有关。

Objective It is to observe the influence of compound Xueshuantong capsule（CXSTC） combined with carbogen on serum inflammatory factors and visual field defects in patients with diabetic retinopathy. Methods 72 cases of diabetic retinopathy were randomly divided into two groups. The control group of 36 patients were treated with dobesilate calcium,and the treatment group of 38 patients were treated with CXSTC combined with carbogen based on the therapy of the control group. After treated for five months,the clinical curative effect were statistied in the two groups,and the changes of visual field defect,fasting plasma glucose（FPG）,2 h postprandial glucose（2h PG）,glycosylated hemoglobin（HbA1c）,plasma homocysteine（Hcy）,cystatin C（Cys C）,high-sensitivity C-reactive protein（hs-CRP）,serum transforming growth factor β1（TGF-β1）,interleukin-6（IL-6） and serum glutathione peroxidase（GSH-Px） were observed before and after treatment in the two groups.Results The total effective rate of the treatment group was significantly higher than that of the control group（P〈0. 05）. After treatment,the gray value of view and hemangioma volume were significantly lower in the two groups（all P〈0. 05）; the bleeding spots area,macular edema was significantly lower in the the treatment group（all P〈0. 05） but lower less in the control group（all P〈0. 05）; the improvement of the horizons gray value,hemangioma volume,bleeding spots area and the reduced degree of macular edema of the treatment group were significantly better than those of the control group（all P〈0. 05）.The levels of FPG,2h PG,Hb A1 c,Cys C,Hcy,TGF-β1and inflammatory factor as hs-CRP,IL-6 in the two groups were significantly reduced（all P〈0. 05） and the levels of the serum GSH-Px were significantly increased（all P〈0. 05）,there were significantly difference between the two groups（all P〈0. 05）. Conclusion The curative effect of CXSTC combined with western medicine in treatment of diabetic retinopathy is very significant,it may be related to suppress the expression of inflammatory factors,and reduce levels of Hcy,TGF-β1,increased GSH-Px.
