布地奈德联合孟鲁司特钠对老年COPD患者肺功能及SIRT1表达的影响

Effect of budesonide combined with montelukast on lung function and SIRT1 expression in elderly patients with chronic obstructive pulmonary disease

目的:探讨布地奈德联合孟鲁司特钠对老年慢性阻塞性肺疾病(COPD)患者肺功能及依赖于烟酰胺腺嘌呤二核苷酸的组蛋白脱乙酰酶(SIRT1)表达的影响。方法:将250例老年COPD患者随机分为两组,观察组采用布地奈德联合孟鲁司特钠治疗,对照组采用布地奈德治疗,比较两组治疗效果和SIRT1蛋白表达的差异。结果:观察组治疗总有效率显著高于对照组(P=0.00);两组患者治疗后生理评分、心理评分及肺功能均显著改善(P<0.05),但观察组改善更明显(P<0.05);两组患者治疗后SIRT1蛋白表达上调(P<0.05),观察组上调更显著(P<0.05);两组患者治疗后各项炎症因子水平显著下降(P<0.05),观察组下降更明显(P<0.05)。结论:布地奈德联合孟鲁司特钠,通过上调SITR1蛋白表达控制气道炎症反应,从而改善肺功能,提高老年COPD患者生活质量,值得临床推广使用。

Objective： To explore the effect of budesonide combined with montelukast on lung function and SIRT1 expression in elderly patients with chronic obstructive pulmonary disease.Methods： Two hundred and fifty elderly patients with chronic obstructive pulmonary disease were randomly divided into observation group and control group.The observation group received budesonide combined with montelukast treatment,while the control group received budesonide treatment only.The therapeutic effects and SIRT1 protein expression were compared between the two groups.Results： The total effective rate in the observation group was significantly higher than that in the control group（ P = 0.00）.Physiological and psychological score and the lung function in the two groups were significantly improved（ P 〈 0.05）,but the magnitude of improvement in the observation group was significantly higher（ P 〈0.05）.SIRT1 expression increased after treatment in the two groups（ P 〈 0.05）,while in the observation group it increased more significantly（ P 〈 0.05）.The level of inflammatory cytokines decreased significantly（ P 〈 0.05）,wheras in observation group it decreased more prominently（ P 〈 0.05）.Conclusion： Budesonide combined with montelukast therapy can improve the life quality and lung function of elderly COPD patients by up-regulating the SITR1 expression to control inflammation,which is worthy of clinical use.
