抗菌药物不同给药时间对手工刮治重度慢性牙周炎短期疗效及牙周膜细胞增生的影响 被引量:3

Short-term effects of antibiotics administered at different times combined with hand scaling on severe chronic periodontitis

摘要 目的:探讨抗菌药物不同给药时间对手工刮治重度慢性牙周炎短期疗效及牙周膜细胞增生的影响。方法:选择在我院治疗的重度慢性牙周炎患者63例,按随机数字表法分为A组(31例)和B组(32例),A组患者在手工刮治的同时给予抗菌药物治疗,B组患者在手工刮治完成后给予抗菌药物治疗。对两组患者牙周检查情况、咀嚼功能、牙槽骨高度、牙周膜细胞增殖情况以及生存质量进行评估。结果:治疗后A组患者平均探诊深度、平均邻面探诊深度、探诊深度超过5 mm位点的百分比、邻面探诊深度>5 mm位点的百分比、平均出血指数、探诊出血位点百分比均显著低于B组(P<0.05),牙槽骨高度显著高于B组(P<0.05),咀嚼效率显著高于B组(P<0.05),生存质量各维度得分均显著高于B组(P<0.05),牙周膜细胞数量在第3、5天均显著高于B组(P<0.05)。结论:刮治同时应用全身抗菌药物有助于抑制牙槽骨高度降低,降低牙周袋深度,提高咀嚼效率,改善患者生存质量,为牙周膜细胞的增殖创造更加适宜的条件。 Objective: To investigate the short-term effects of antibiotics administered at different time points combined with hand scaling on severe chronic periodontitis and periodontal ligament cell proliferation.Methods:Sixty-three patients with severe chronic periodontitis treated in our hospital were randomly divided into A group( n = 31) and B group( n = 32) according to random number table.The patients in the group A received manual curettage and antibiotic treatment simultaneously,while the patients in the group B received antibiotic therapy after manual curettage.The masticatory function,alveolar bone height,periodontal ligament cell proliferation and life quality of the two groups were evaluated.Results: After the treatment,the probing depth,average adjacent surface probing depth,percentage of probing depth and interproximal probing depth of more than 5 mm loci,average bleeding index and probing bleeding site percentage in group A were significantly lower than those in group B( P 〈0.05).The alveolar bone height in group A was significantly higher than that in group B( P 〈 0.05).After treatment,the masticatory efficiency and life quality scores in group A were significantly higher than those in group B( P 〈 0.05).The number of periodontal ligament cell of group A on the 3rd and 5th day was significantly more than that of group B( P 〈 0.05).Conclusion: Scaling and application of systemic antibiotics simultaneously can inhibit the decline of alveolar bone height,reduce the exploration of probing depth,improve the chewing efficiency and life quality,and provide a better condition for the proliferation of periodontal ligament cells.
