摘要
目的:探讨铜绿假单胞菌耐药率与抗菌药物用药频度(defined daily dose system,DDDs)之间的关系,为临床合理使用抗菌药物提供参考。方法:对河南省胸科医院2013年7月—2015年6月各季度抗菌药物的DDDs及所分离出的铜绿假单胞菌的药物敏感性试验结果分别进行统计分析。结果:铜绿假单胞菌对注射用哌拉西林钠他唑巴坦钠的耐药率与注射用头孢他啶的DDDs呈正相关(r=0.767,P<0.05);铜绿假单胞菌对注射用头孢他啶的耐药率与硫酸阿米卡星注射液、左氧氟沙星注射液的DDDs呈正相关(r值分别为0.892、0.772,P<0.05);铜绿假单胞菌对注射用美罗培南的耐药率与注射用头孢他啶的DDDs呈正相关(r=0.733,P<0.05);铜绿假单胞菌对左氧氟沙星注射液的耐药率与注射用头孢他啶、注射用亚胺培南西司他丁的DDDs呈正相关(r值分别为0.718、0.771,P<0.05)。结论:铜绿假单胞菌的耐药率变化与抗菌药物的DDDs之间存在着一定的相关性,随着抗菌药物DDDs的增加,铜绿假单胞菌对相关抗菌药物的耐药率也会增加。定期监测所在地区抗菌药物的DDDs和铜绿假单胞菌的耐药率,对于临床治疗铜绿假单胞菌引起的感染具有重要意义。
OBJECTIVE: To probe into the correlation between resistance of pseudomonas aeruginosa and defined daily dose system( DDDS) of antibiotics,so as to provide reference for the rational application of antibiotics in clinic.METHODS: Statistical analysis was respectively conducted on DDDs of antibiotics in each quarter and sensitivity test results of the isolated pseudomonas aeruginosa in Henan Provincial Chest Hospital from Jul. 2013 to Jun. 2015.RESULTS: The resistance of pseudomonas aeruginosa to piperacillin tazobactam for injection was positively related with the DDDs of ceftazidime( r = 0. 767,P〈0. 05); the resistance of pseudomonas aeruginosa to ceftazidime was positively correlated with the DDDs of amikacin injection and levofloxacin injection( r were 0. 892 and 0. 772 respectively,P〈0. 05); the meropenem's resistance to pseudomonas aeruginosa was positively related with the DDDs of ceftazidime( r =0. 733,P〈0. 05); the resistance of pseudomonas aeruginosa to levofloxacin injection was positively correlated with the DDDs of ceftazidime and imipenem / cilastatin( r were 0. 718 and 0. 771 respectively,P〈0. 05). CONCLUSIONS:The resistance transition of pseudomonas aeruginosa has a relationship with the antibiotics consumption. Increased as the DDDs of antibiotics,the resistance of pseudomonas aeruginosa to relevant antibiotics would increase. Therefore,to monitor the application of antibiotics and the resistance of pseudomonas aeruginosa regularly in the region will make a significant difference on the treatment of pseudomonas aeruginosa infection.
